Per Florida Statute 112.3143,I hereby disclose by written memorandum that I will abstain from
the vote on certain issues that are brought before the Monroe County Board of Commissioners with entities
that I am involved with.
I will abstain from the vote on issues concerning the Guidance Care Center,Inc.,a private,not-
for-profit entity,which receives some of its operational funding from the County,as I am currently a
member of the Board of Directors of the Guidance Care Center.
At the December 14, 2011, BOCC meeting,I will abstain from the vote on item(s).
#C21 Approval of amendment to contract with the Guidance Care Center,replacing
Attachment D, which concerns subcontracted Baker/Marchman Act transportation.
• #01 Adoption of a resolution supporting the continuation of coordinated Medicaid
transportation services as they are currently being provided in Monroe County,Florida and
discouraging implementation of Medical Reform legislation which would merge these services
into HMO's that currently provide no medical services in the Keys.
