ITEM BACKGROUND: The purpose of this Amendment is to increase the amount under contract for the 9
month period ending December 31, 2006 by $5,722.00 for a total of$383,443.00. This increase is non-
recurring and will not affect the contract allocation in subsequent periods.
PREVIOUS RELEVANT BOCC ACTION: Approval of Amendment #001 to OAA Contract #AA-629 on
May 16, 2006.
CONTRACT/AGREEMENT CHANGES: Allotted funds now are Title IlI-B $79,600.00; Title C-I
$118,950.00, Title C-II $138,592.00 and Title III-E $46,301.00

THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and Monroe County Board of Commissioners.
The purpose of this amendment is to increase the amount under contract for the 9 month
contract period ending December 31,2006 by $ 5,722 for a total of $ 383,443. This increase

breakdown by Title will be as following:
Title III B: $ 79,600 /
Title C1: $118,950
Title C2: $ 138,592
Title III E: $ 46,301
1. This amendment shall begin on April 1 , 2006 or the date it has been signed by both parties,
whichever is earlier.
All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the contract are hereby amended to conform with this amendment.
This amendment and all its attachments are hereby made a part of the contract.

"recipient." This agreement is subject to all provisions contained in the MASTER
AGREEMENT executed between the Alliance and the Recipient, Agreement No. P A- 429, and
its successor, incorporated herein by reference.
