AGENDA ITEM WORDING: Request approval of Amendment 002 to the Nutrition Services
Incentive Program (NSIP) Contract US-951 between the Board of County Commissioners and the
Alliance for Aging, Inc. This amendment increases the reimbursement rate per meal paid to the County
for the period 9/1/09-9/30/09.
ITEM BACKGROUND: This contract allows for reimbursement to the County for congregate and
home delivered meals provided to clients of the Monroe County Nutrition Program. The Alliance for
Aging periodically changes the reimbursement rate based upon the availability of funds.
PREVIOUS RELEVANT BOCC ACTION:
Approval of Amendment 001 to this contract US-951 on August 19, 2009.
CONTRACT/AGREEMENT CHANGES:
Increases the reimbursement rate for the period 9/l/09 — 9/30/09 from .6295267 to .6395316. The total
dollar amount of the contract remains the same.

THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and Monroe County Board of Commissioners.
The purpose of this amendment is to change the unit rate. Payments will be made based on
availability of funds.
This is a fixed rate contract. The Alliance shall make payment to the recipient for provision
of services up to a maximum number of units of service and at the prospective rate stated
below'

3. Final Request for Payment:
The Recipient must submit the final request for payment to the Alliance by October 30,
2009; if the Recipient fails to do so, all right to payment is forfeited, and the Alliance
will not honor any requests submitted after the aforesaid time period.
All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the contract are hereby amended to conform with this amendment.
This amendment and all its attachments are hereby made a part of the contract.
