A RESOLUTION BY THE BOARD OF COUNTY
COMMISSIONERS OF MONROE COUNTY, FLORIDA,
AUTHORIZING THE MAYOR TO APPROVE AN
ADDENDUM TO THE GENERAL REVENUE/FEDERAL
FUNDS CONTRACT BETWEEN MONROE COUNTY
AND THE UNITED WAY OF MONROE COUNTY
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that the Mayor/Chairman of the Board
is hereby authorized to approve an addendum to the general
revenue/federal funds contract, a copy of same being attached
hereto, between Monroe County and the United Way of Monroe
County concerning six (6) leased vehicles (described within).
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held

ADDENDUM
TO
GENERAL REVENUE/FEDERAL FUNDS CONTRACT
BETWEEN
THE UNITED WAY OF MONROE COUNTY
AND
THE MONROE COUNTY BOARD OF COUNTY COMMISSIONERS
OF
JANUARY 31, 1989
This ADDENDUM authorizes a second lien interest of twenty percent
(20%) to the Monroe County Board of County Commissioners, herein
referred to as the "Provider" for six (6) leased vans (described
herein) jointly funded by:
STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION
DIVISION OF PUBLIC TRANSPORTATION OPERATIONS
l6(b)(2) JOINT PARTICIPATION AGREEMENT;
and limits the financial responsibility of the UNITED WAY OF MONROE
COUNTY, herein referred to as the "Agency" to the Provider to the
physical property of the six (6) leased vans (described herein).
Mr. Mike Derbonne of the Florida Department of Transportation
(FDOT) authorized the additional encumbrance under WPI No.:68l90l2
subpara 13.50 for a public agency on April 24,1989.
WITNESSETH: For the Provider to secure and interest in the
amount of $29,778.00 as follows:
for partial payment for the purchase of six (6) 1989
White Ford Raised Roof Econoline Vans as ~scribed in the
attached Schedule.
The Agency agrees that the Provider is the second lien holder to
said vans.
The Provider will have a twenty percent (20%) interest in said vans
effective immediately upon payment of $29,778 to the Agency by the
Provider.
The terms, warranties and agreements herein, contained shall bind
and inure to the benefit of the respective parties hereto, and
their respective legal representatives, successors and assigns.

The gender and number used in this agreement are used as a
reference term only and shall apply with the same effect whether
the parties are of the masculine or feminine gender, corporate or
other form, and the singular shall likewise include the plural.
This agreement may not be changed orally.
IN WITNESS WHEREOF, the parties have respectively signed and sealed
these presents the day and year first written above.
