RESOLUTION NO. 133-1986
A RESOLUTION RESCINDING RESOLUTION NO. 111-
1986 AND AUTHORIZING THE MAYOR AND CHAIRMAN
OF THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, TO EXECUTE AN AD-
DENDUM TO AGREEMENT FOR BAKER ACT TRANSPOR-
TATION FUNDS BY AND BETWEEN THE BOARD OF
COUNTY COMMISSIONERS OF MONROE COUNTY,
FLORIDA, AND THE GUIDANCE CLINIC OF THE
MIDDLE KEYS.
WHEREAS, the Board of County Commissioners of Monroe County,
Florida, previously adopted Resolution No. 111-1986 on April 4,
1986, and said Board now desires to rescind said Resolution as it
erroneously referred to Baker Act Matching Funds instead of Baker
Act Transportation Funds, and
WHEREAS, said Board now desires to approve and authorize
execution of an Addendum to Agreement for Baker Act Transporta-
tion Funds, now, therefore,
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, as follows:
1. That Resolution No. 111-1986 is hereby rescinded.
2. That the Mayor and Chairman of the Board of County
Commissioners of Monroe County, Florida, is hereby authorized to
execute an Addendum to Agreement for Baker Act Transportation
Funds by and between the Board of County Commissioners of Monroe
County, Florida, and the Guidance Clinic of the Middle Keys,
Inc., a copy of same being attached hereto.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held
on the 18th day of April, A.D. 1986.

ADDENDUM TO AGREEMENT FOR
BAKER ACT TRANSPORTATION FUNDS
THIS ADDENDUM, made this
18th
day of
April
1986, by and between the BOARD OF COUNTY COMMISSIONERS OF MONROE,
FLORIDA, hereinafter referred to as "BOARD", and the GUIDANCE
CLINIC OF THE MIDDLE KEYS, INC., hereinafter referred to as
"CLINIC".
For and in consideration of the provisions of transportation
to treatment facilities as required by Florida Statutes, ~394.459
for Monroe County patients, the parties hereby agree as follows:
(1) To amend paragraph 2. of "1ts Agreement entered into on
January 24, 1986, copy attached hereto, by adding to the amount
payable by the BOARD a sum not to exceed $14,000 for the
providing of transportation of patients in Monroe County to
treatment facilities, for a total of $38,40S.00.
IN WITNESS THEREOF, the BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, has caused these presents to be signed in
its name by its Chairman, Board of County Commissioners, and the
GUIDANCE CLINIC OF THE MIDDLE KEYS, INC., has caused these
presents to be signed in its name by its Clinic Director and
President of the Board, all as hereinabove set forth.
