RESOLUTION NO. 219 -1985
A RESOLUTION AUTHORIZING THE CHAIRMAN OF THE
BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, TO APPROVE AND EXECUTE AN
AGREEMENT BY AND BETWEEN GEORGE CHAKMAKIS AND
THE BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, CONCERNING PRE-EMPLOYMENT
PHYSICALS FOR PROSPECTIVE AND CURRENT
EMPLOYEES.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that the Chairman of the Board of County
Commissioners of Monroe County, Florida, is hereby authorized to
approve and execute an agreement by and between George Chakmakis
and the Board of County Commissioners of Monroe County, Florida,
a copy of which is attached hereto, concerning pre-employment
physicals for prospective and current employees.
PASSED AND ADOPTED by the Chairman of the Board of County
Commissioners of Monroe County, Florida, at a regular meeting of
said Board held on the 19th day of July, A.D. 1985.

1985, by and between the BOARD
MONROE COUNTY, FLORIDA, party of the firs t part, and GEORGE
CHAKMAKIS, M.D., party of the second part, whose mailing address
is Key Largo Medical Clinic, P.O. Box 430, Key Largo, FL 33037.
WITNESSETH, that the party of the second part, for and
in consideration of the sum of $25.00 per patient, payable upon
receipt of invoice, agrees to furnish pre-employment physicals
for prospective employees and any physicals deemed necessary for
current employees of the party of the first part.
It is hereby further agreed by and between the parties
hereto that either party may cancel this agreement by giving
thirty (30) days notice in writing.
