RESOLUTION NO. 151 -1983
RESOLUTION AUTHORIZING THE CHAIRMAN OF THE BOARD OF
COUNTY COMMISSIONERS OF MONROE COUNTY, FLORIDA, TO
EXECUTE AN AGREEMENT FOR BAKER ACT MATCHING FUNDS
BY AND BETWEEN THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, AND THE DADE-MONROE MENTAL
HEALTH BOARD, INC., DISTRICT XI.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, as follows:
That the Mayor and Chairman of the Board of County Commis-
sioners of Monroe County, Florida is hereby authorized to execute
an Agreement for Baker Act Matching Funds by and between the Board
of County Commissioners of Monroe County, Florida, and the Dade-
Monroe Mental Health Board, Inc., District XI, a copy of same being
attached hereto.
Passed and adopted by and between the Board of County Commis-
sioners of Monroe County, Florida, at a regular meeting of said
Board held on the 27th day of May, A.D. 1983.

_AGREEMENT FOR BAKER•ACT MATCHING FUNDS

AHIS AMENDMENT., effective this 2 7 th day of May , 1983, by
and between the BOARD OF COUNTY COMv1ISSLONERS. OF MONROE COUNTY, FLORIDA, .hereinafter
referred to.as "BOARD", and the DADE-MONROE MENTAL HEALTH BOARD, INC. , District XI,
hereinafter referred to as "DISTRICT", implements the. following changes to the '
original agreement entered into on December 13, 1982.
(1) • The Board agrees to reimburse the District twenty-five percent (25%) of
total District expenditures for Baker act hospital, physician, and crisis stabilizati
• . services, as billed to the District for clients qualifying for such services in
• accordance with the Rules of the Department of Health and Rehabilitative Services
Alcohol, Drug Abuse and Mental Health Program Office Chapters 10E-:5, 10E-12
10E-13and 10E-14. This cost is not to exceed a total reimbursement of Forty-Five
Thousand Six Hundred and Eighty Four Dollars ($45,684) during.the period begining
October 1, 1982 and ending.September 30, 1983. .

This one (1) page. Amendment is effective for the entire contract period.
All other terms and conditions remain in full force and effect.
