RESOLUTION NO. 538 -1989
A RESOLUTION OF THE BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, AUTHORIZING
THE MAYOR/CHAIRMAN OF THE BOARD TO EXECUTE
AN ADDENDUM TO AGREEMENT BETWEEN MONROE COUNTY
AND USA 800, INC. CONCERNING BULK REQUESTS.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that the Mayor/Chairman of the Board is
hereby authorized to execute an Addendum to Agreement between
Monroe County and USA 800, Inc., a copy of same being attached
hereto and made a part hereof, concerning bulk requests.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held

ADDENDUM to AGREEMENT
By signing this addendum, both parties agree to install the
following items into the body of the original agreement. The
charges on page (2) of the original agreement are still in effect
for consumer and trade requests. This addition is for bulk
requests only that were not included in the original agreement.
BULK REQUESTS:
Calls per month.
Take name, company name, address, city, state, zip, business
phone, ad source, requested quantity and intended use and send
out fulfillment package (up to 5 pes.).
1 - 2000........................$ 1.65
2001 - 4000........................$ 1.60
4001 - 6000........................$ 1.55
6001 - & UP........................ $ 1. 50
Data Entry.
USA 800 will input all of the above information from mail-in
requests received at our office and send out fulfillment package
(up to 5 pieces).
Cost per entry..................... $ . 30
Packaging.
USA 800, Inc. will bundle brochures to consist of (25) pieces per
bundle. * This will be done on a regular basis and be invoiced as
a special handling charge when incurred.
Cost per bundle....................$ .12
* Each bundle shall constitute (1) piece in fulfillment pack _ *
* Any amounts requested not in increments of 25 will be charged
at the contracted price of three cents .each.

USA 800, shall at their discretion, mail out the requested
brochures based on the reasonableness of the request and
conformi ty to the guidelines set forth by the administrator's
office.
USA 800 shall provide the administrator's office a listing of all
special requests on a monthly basis.
All requests shall appear on all existing reports to reflect
captured information.
USA 800 shall be authorized to ship requested information via UPS
if it is the most cost effective carrier.
All program modification costs will be charged at the prescribed
rates outlined in current agreement.
