AUTHORIZING THE MAYOR TO APPROVE THE
AMENDMENT TO CCE CONTRACT KG052, BETWEEN
MONROE COUNTY/SOCIAL SERVICES (IN HOME
SERVICES) DEPT. AND THE DEPARTMENT OF
HEALTH AND REHABILITATIVE SERVICES.

BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, hereby authorizes the Mayor to approve
the amendment to contract KG052, between Monroe County/Social
Services (In Home Services) Dept. and the Department of Health
and Rehabilitative Services, a copy of same being attached
hereto.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held

THIS A~ENDME~T, entered into between the St~te of ?lorida, Department
of Health and Rehabilitative Services, hereinafter referred to as the
"Department", ar:d Monroe County (In-Home Services), herelnafter re~erred
to as the "Prov~der" aMends Contract KGC5";t
1. Section II, Subsection A (Contract Arnou~t) is hereby amended to
read:
To pay for contracted services according to the conditions of
Attachment I, in an amount not exceed $538.096.00 subject to
the availability of dunds. The State of Florida's ?erfor~a~ce
and obligation to pay under this Contract is contingent upon
an annual appropriation by the Leaislature. The cost of servi-
ces paid under any other Contract or froM any other are not
eligible for reirr.bursement ~r.der this Contract.
2. Attachment I, s~ction C, Paragraph 1 (Method of Pa~.ent) is
hereby amended to read:
1. Subject to the availability of funds, the ~epartment will
reimburse no more than a total dollar amount of$538,Ogh.OO
from general revenue for expenditures made in accordar.ce with
the a?proved applicat{on for Community Care for the Elcerly
funds.
3. Attachment I, Section D, Para~raoth 1, (Match) is hereby amended
to read:
1. Total Provider match for this contract period will be at
least $.59.7.88.44 . The provider' s contribution will be
made in the form of cash and/or in-kind resources. Bv the
end o~ the sixth month of the contract period matching amounts
provided must be in proper proportion to the Co~~unity Care
for the Elderly funds expended. At the end of the contract
period, all Community Care :or the Elderly funds expended must
be properly matched.
4. This Amendment shall begin on 3/1/90 or the date on which the
Amendment has been signed by both parties, whichever is later.
All provisions in the Contract and any Attachments thereto in
conflict with this Amendment are hereby changed to conform with
this Amendment.
All provisions not in conflict with this Amendment are still in
e:fect-and are to be performed at the level s?ecified in the
Contract.
