At the HaCC meeting held on June 20, 2007, the Board approved the following:
Ratification of the County Administrator signature authority for an amendment to
the Health Care for the Elderly (HCE) Contract KH672 between the Alliance for Aging,
Inc., and the BaCC that increases service capacity during the month of June 2007.
Attached hereto is a duplicate original of the subject document for your handling.

THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and Monroe County Board of Commissioners ("Monroe').
The purpose of this amendment is to increase the amount under contract for Case
Management to $10,309 and to clarify that the $51,544 referred to in the contract is now
comprised of $ 10,309 for Case Management and $41,235 for Subsidy. The Subsidy amount
is listed for reference purposes only as the Alliance is responsible for issuing the Subsidy
paymenlts.

1. This amendment shall begin on July 1, 2006 or the date it has been signed by both parties,
whichever is earlier.
All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the contract are hereby amended to conform with this amendment.
This amendment and all its attachments are hereby made a part of the contract.
