approval and authorized execution of Amendment 0001 to 99-00 Community Care for Disabled
Adults Contract #KG-026 between the State of FloridalDepartment of Children & Families and
Monroe County Board of County CommissionersIMonroe County Social Services (Monroe
County In Home Service Program, the case management agency).
Enclosed please find three duplicate originals executed on behalf of Monroe County for
your handling. Please be sure that the fully executed "Clerk's Orginal" is returned to our

AMENDMENT #0001
THIS AMENDMENT, entered into between the State of Florida, Department of Children
and Families, hereinafter referred to as the "department", and Monroe County In Home Services,
hereinafter referred to as the "provider", amends contract KG026.
1. Standard Contract, Section II, paragraph A. is hereby amended to read:
To pay for contracted services according to the conditions of Attachment I in an amount not to
exceed $139,639.12, subject to the availability of funds. The State of Florida's performance and
obligation to pay under this contract is contingent upon an annual appropriation by the Legislature.
The costs of services paid under any other contract or from any other source are not eligible for
reimburs'lment under this contract.
2. Standard Contract, Section III, paragraph A. is hereby amended to read:
This contract shall begin on July I, 1999, or the date on which the contract has been signed,
whichever is later. It shall end on September 30, 2000.
3. Attachment I, Section B. Manner of Service Provision, paragraph S.d. is hereby amended to read:
d. The contract period is defined as July 1, 1999 through September 30, 2000.
4. Attachment I, Section C. Method of Payment, paragraph 2. is hereby amended to read:
2. The total dollar amount ofthis contract is $139,639.12. The department shall make
payment to the provider for a total dollar amount not to exceed $139,639.12, which is 90%
of the total cost and only subject to the availability of funds.
5. Attachment I, Section C. Method of Payment, paragraph 3. is hereby amended to include the

maximum number of units of service and at the rates stated below:

6. Attachment I, Section C., paragraph 6. is hereby amended to read:
6. The provider match for this contract is $15,515.57. Case management and/or
transportation may be exempt from this requirement at the discretion of each district.
7. This amendment shall begin on June 30, 2000, or the date on which the amendment has been
signed by both parties, whichever is later.
8. All provisions in the contract and any attachments thereto in conflict with this amendment shall be
and are hereby changed to conform with the amendment.
9. All provisions not in conflict with this amendment are still in effect and are to be performed at the
level specified in the contract.
10. This amendment and all its attachments are hereby made a part of this contract.
