062 - 2008
A RESOLUTION OF THE BOARD OF COUNTY
COMMISSIONERS OF MONROE COUNTY, FLORIDA,
APPROVING NEW YORK LIFE INSURANCE COMPANY TO
OFFER VOLUNTARY SUPPLEMENTAL AND DEPENDENT
LIFE INSURANCE TO COUNTY PERSONNEL.
WHEREAS, Monroe County does not currently offer supplemental life or dependent
life insurance for its personnel; and
WHEREAS, some individuals, at their own expense, may desire to possess
additional life insurance coverage for specific needs; and
WHEREAS, Monroe County received proposals to provide supplemental life and
dependent insurance products to personnel and has selected New York Life Insurance
Company as the provider; and
WHEREAS, there is no employer contribution to fund this benefit. New York Life
makes arrangements with the individual to direct the withdrawal and transfer of funds from
the individual's designated account, eliminating the County's involvement in the
administrative process.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA
That New York Life Insurance Company shall be offered as the voluntary supplemental life
and dependent insurance provider for county personnel and the county shall assist in
coordinating enrollment meetings with the provider.
PASSED AND ADOPTED by the Board of County Commissioners of Monroe County,
Florida, at a regular meeting of said Board held on the 1..9.thJday of M,qrrn
