RESOLUTION AUTHORIZING THE CHAIRMAN OF THE BOARD OF
GOVERNORS OF MUNICIPAL SERVICE TAXING DISTRICT NO. Z
TO EXECUTE AN AGREEMENT BY AND BETWEEN MUNICIPAL SER-
VICE TAXING DISTRICT NO.2 AND FISHERMEN'S HOSPITAL,
INC. FOR AMBULANCE SERVICE.
BE IT RESOLVED BY THE BOARD OF GOVERNORS OF MUNICIPAL
SERVICE TAXING DISTRICT NO.2 as follows:
That the Chairman of the Board of Governors of Municipal
Service Taxing District No. 2 is hereby authorized to execute an
Agreement by and between Municipal Service Taxing District No. 2
and Fishermen's Hospital, Inc., a copy of same being attached
hereto, for ambulance service.
Passed and adopted by the Board of Governors of Municipal
Service Taxing District No.2, at a regular meeting of said Board
held on the 10th day of March, 1981.

1981, by and between MUNICIPAL SLRVICE TAXING DISTRICT :iO 2
governed by the Board of ,:ounty Commissioners of Monroe (\mnty,
florida, and City CommissIoners Jf the City of Key Colony Beach,
Florida, hereinafter called "THE DISTRICT" and FISHERMEN'S
HOSPITAL, INC., a non-profit Florida corporation, hereinafter ~alled
"THE HOSPITAL".
WHEREAS, THE DISTRICT and TilE HOSPITAL desire to operate an
ambulance service fro~ THE HOSPITAL premises in Marathon, Florida,
to service the area generally describe~ as being between Little
Duck Key, approximate Mile Marker 39, and Long Key 8ridge, approxi-
mate Mile Marker 66, including the City 0; Key Colony BI Bch,
NOW, THEREFORE, upon the following terms and condi lions ,~HE
DISTRICT a~d THE HOSPITAL agree to commence operation ot the abcv2
described ambulance service:
1. THE DISTRICT agrees to reimburse to THE IIl1SPIT."L all costs,
charges or expenditures necessary or incidental to th2 operation 01
the ambulance ser,ice, which costs, expenditures or charges are liOt
actually received by THE HOSPITAL from patients as payme'lt f01' the
ambulance services not to exceed the sum of Forty-four Thousand
Five Hundred Dollars ($44,500.00).
THE HOSPITAL shall use s~id
money solely for the operation of the ambulance service.
t. THE HOSPITAL in its sole discretion sh~ll set the fees to
.be charged for the ambulance service and THE HOSPITAL shall have tt
sole authority and respon~ibility to render statements to patIents
for services rendered by the ambulance service. THL HOSPITAL shall
have the sole authority to deterlliine the procedures ~o be useu in
the colle~t~on of fees for the use of the ambulance ~erVlce anJ
THE HOSPI'If\L shall not be obligated to ill,tiate any lawsuit OT other
procedure in order to collect said fee.
All fees charged by THE
HOSP_TAL f~r the use of the ambu]~nce service shall be credited
against the costs .)f the operation of the said ambulanc2 seYv.c~.

3. rHE HOSPITAL shall keep accurate records of all costs
paid for the operation or benefit of the ambulance service and
THE HOSPITAL shall also make accurate records of all fees charged
for the use of the ambulance service. A separate record shall be
kept for all fees actually recovered by THE HOSPITAL in payment for
the ambulance service.
4. THE HOSPITAL will provide monthly financial statements of
the operation of the ambulance service. If, after examination of
said monthly statements a deficit occurs, THE DISTRICT WIll reim-
burse THE HOSPITAL for said deficit in an amount not to exceed the
amount of this contract.
S. rHE DISTRICT agr~es to lease to THE HOSPITAL for the sum
of One and 00/100 Dollars ($1.00) per year the existing county
equipment owned by THE DISTRICT per Attachment A hereto.
6. THE HOSPITAL shall furnish all necessary personnel to
operate the ambulance service on a twenty-four (24) hour basis.
THE HOSPITAL shall further maintain all ambulance service equipment.
All costs of THE HOSPITAL for such personnel and maintenance shall
be a cost within the meaning of this Agreement. Additionally, THE
HOSPITAL shall provide insurance on the personnel operating the
ambulance and insurance on the ambulance in order to protect THE
DISTRICT from any public or private liability. The COSL of the
insurance shall be a cost, charge or expenditure of the dmbulance
service within the meaning of this Agreement.
7. The term of this Agreement shall commence on the ls~ day
of October, 1980, and end on the 30th day of September, 1981.
