At the November 19,2008 Board of County Commissioner's meeting, the Board granted
approval and authorized execution of Amendment 005 to the Older Americans Act (OAA)
Contract #AA-829 (01/01/2008 to 12/31/2008) between the Alliance for Aging, Inc. (Area
Agency on Aging for Miami-Dade and Monroe Counties) and the Monroe County Board of
County Commissioners (Community Services/In-Home and Nutrition Services).
Enclosed are three duplicate originals of the above-mentioned for your handling. Please
be sure that the sets stamped Monroe County Clerk's Office Original and Monroe County
Finance Department's Original are returned to my attention as quickly as possible. Should you

On November 19, 2008, the Board of County Commissioners approved and authorized execution of Amendment
005 to the Older Americans Act (OAA) Contract #AA-829 between Monroe County and the Alliance for Aging,
Inc.
Enclosed are two (2) originals that are fully executed. One is the Monroe County Finance Department's Original

THIS AMENDMENT is entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and Monroe County Social Services Division.
The purpose of this amendment is to decrease Title C1 by $6,000 thereby decreasing the
OAA Title C1 contract amount to $ 142,600 and increase Title C2 by $6,000 thereby increase
the OAA Title C2 contract amount to $ 210,832.The total OAA contract will be $519,561. The
unit rate is not amended therefore the provider must adjust the number of units of services
to be provided under the affected title accordingly.
1. Section II , Paragraph C, Sources of Funds, is hereby amended to read:

2. This amendment shall begin on January 1, 2008 or the date it has been signed by both
parties, whichever is earlier.
All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the contract are hereby amended to conform with this amendment.
This amendment and all its attachments are hereby made a part of the contract.
IN WITNESS WHEREOF, the parties hereto have caused this amendment to be executed by their
undersigned officials as duly authorized.
