At the February 18, 2009, Board of County Commissioners' meeting the Board granted
approval and authorized execution of Amendment #001 to the Alzheimer's Disease Initiative
(ADI) Contract No. KZ897 between the Alliance for Aging, Inc. (Aging Resource Center) and
the Monroe County Board of County Commissioner (Community Services/In-Home Services)
for Fiscal Year 7/1/08 to 6/30/09.
Enclosed are three duplicate originals of the above-mentioned, executed on behalf of
Monroe County, for your handling. Please be sure to return the fully executed "Monroe County
Clerk's Office Original" and the "Monroe County Finance Department's Original" as soon as

On February 18, 2009, the Board of County Commissioners approved and authorized execution of Amendment
001 to the Alzheimer's Disease Initiative (ADO Contract KZ-897 between Monroe County and the Alliance for
Aging, Inc.
Enclosed are two (2) originals that are fully executed. One is the Monroe County Finance Department's Original
and the other is a blank Original.

THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "A.lliance", and Monroe County Board of Commissioners
The purpose of this amendment is to decrease the amount under contract by $6,155 from
$65,888 to $59,733. This decrease includes a 40/0 holdback from DOEA
The provider must adjust the Unit Cost Methodology to take this decrease in funding into
accou nt.
METHOD OF PAYMENT:
1. This amendment shall begin on July 1, 2008 or the date it has been signed by both parties,
whichever is earlier.
All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the contract are hereby amended to conform with this amendment.
This amendment and all its attachments are hereby made a part of the contract. This
amendment and all its attachments are hereby made a part of the contract.
