At the June 17, 2009, Board of County Commissioners' meeting the Board granted
approval and authorized execution of Amendment 001 to Contract AA-929 between Monroe
County Social Services and the Alliance for Aging, which changes the unit rate and the number
of service units for Nutrition Education. The total amount of funding for Nutrition Education
remains the same.
Enclosed are two duplicate originals of the above-mentioned, executed on behalf of
Monroe County, for your handling. Please be sure to return the fully executed "Monroe Couty
Clerk's Office Original" as well as the "Monroe County Finance Department's Original" as soon
as possible. Should you have any questions, please do not hesitate to contact our office.

THIS AMENDMENT is entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and "Monroe County Social Services".
The purpose of this amendment is to adjust the Unit Rate and number of units for Nutrition
Education.
1. The Alliance shall make payment to the provider for provision of services up to a maximum
number of units of service and at the rate( s) stated below:

2. This amendment shall begin on January 1, 20<JCt or the date it has been signed by both

All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
