Enclosures CC: Sheryl Graham, Director, Social Services Amendment 005 CONTRACT KH 972 Page 1 THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred to as the "Alliance", and Monroe County Board of Commissioners. The purpose of this amendment is to increase the amount under contract for Case Management by $1,466 from $10,721 to $12,187, to cover 2 new referrals and adjust the annual number of units per client. The provider must adjust the Unit Cost Methodology to take this increase in funding into account. METHOD OF PAYMENT: 1. This amendment shall begin on July 1, 2009 or the date it has been signed by both parties, whichever is later. All provisions in the contract and any attachments thereto in conflict with this amendment shall be and are hereby changed to conform with this amendment. All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the contract are hereby amended to conform with this amendment.
