On January 18, 1995, the Board of County Commissioners granted approval and
authorizedl execution of the following:
Change Order No. 27-001-004 to D. L. Porter Construction's Contract No. FB-
001, Construction, for the Big Coppitt Fire Facility, in the amount of $2,884.00,to
resolve the storm water drainage issues presented by the Homeowners Association.
Amlendment No.1 to Contract M-OOI, between Monroe County and Professional
Service Industries, Inc., to renew their annual services agreement for threshold
inspection, quality control testing and subsurface investigation, for a period of
one year beginning December 22, 1994.
Attached hereto for your handling is a duplicate original of the above documents,
now sealed and executed by all parties.

payment these e:l(tra costs for this completed change order work on dai ly force account reports in
accordance with Paragraph 12.1.4 of the Contract Supplementary General Conditions.
