Dear Ms. Peacock:
Professional Service Industries, Inc (PSI) is pleased to submit this proposal for Construction
Materials Testing and Inspection Services for the referenced project. This proposal outlines our
understanding of the construction project and presents our proposed estimated total fee for the
foundation, building and pavement components. All services provided by PSI will be performed
under the guidelines of the Monroe County Board of County Commissioners' Agreement Between
Owner and Consultant, dated December 22, 1993.
CONSTRUCTION MATERIALS TESTING SERVICES
PSI proposes to provide testing services which include foundation, reinforced concrete, masonry
and pavement. Specific testing services related to the foundation, building and pavement
components are as follows:
SOILS: Review Geotechnical report, monitor augercast pile installation, provide
compaction tests and related laboratory tests on soils.
CONCRETE: Verify placement of reinforcing steel prior to concrete placement. Test
slump, temperature and air content of fresh concrete. Monitor placement
and elapsed time between batching and placement of concrete. Cast and

Total fees will be determined by the actual amount of technical time expended for this project and
the actual quantity of laboratory tests performed. Should items be included which are not desired,
the scope of services may be modified accordingly.
The attached estimate was developed by reviewing project plans and specifications and by
comparing this project to other similar projects. Accordingly, we suggest that a budget of
$14,000.00 be established for testing of this project. This estimated fee does not include overtime
hours or field or laboratory tests required to re-evaluate work or materials initially found to be
out of compliance with project specifications.
The actual amount of testing services required for this project, and therefore the total fee, is
dependent upon numerous factors including the performance of the general contractor,
subcontractors and their material suppliers. These and other not -mentioned factors are beyond
PSI's control, therefore, the estimate presented in this proposal is for budgetary purposes only.
PSI will proceed with the work on this project with oral authorization, however, a signed copy
of this proposal must be returned prior to reports being issued. A copy of PSI's General
Conditions are attached, and are therefore incorporated as part of the proposal. When returning
this proposal, please complete the attached Project Data Sheet for our files.
