At the June 15, 2011, Board of County Commissioner's meeting the Board granted approval and authorized execution of the following: Item C4 Extend existing contract with Hospice of the Florida Keys, Inc. d/b/a Visiting Nurse Association to provide direct care services (Chore, Homemaking, Personal Care, and Respite). Item C5 Amendment 002 to the Visiting Nurse Association (VNA) Contract (7/l/11-12/31/11) between Monroe County Board of County Commissioners (Social Services/In-Home Services) and Hospice of the Florida Keys, Inc. d/b/a Visiting Nurse Association. Should you have any questions, please do not hesitate to contact our office. cc: County Attorney Finance File VISITING NURSE ASSOCIATION AMENDMENT #002 CONTRACT - FY: 7-1-10 TO 12-31-11 THIS AMENDMENT, entered into between the Monroe County Board of County Commissioners (Social Services/In-Home Services Program) (hereinafter called the -Owner" or "County" and Visiting Nurse Association (VNA), hereinafter referred to as the "contractor" amends the Visiting Nurse Association (VNA) contract for FY: 7-1-10 to 12-31-11. A. Collaboration with Memory Disorder Clinics: Memory Disorder Clinics are required to provide four (4) hours of Alzheimer's Disease in- service training to all Respite workers in their designated service areas. The Contractor agrees to collaborate with Memory Disorder Clinics to assist in this effort. B. Prioritization for Service Delivery for Abuse, Neglect and Exploitation (APS) clients The Contractor will ensure that pursuant to Section 430.205(5), Florida Statute, those elderly persons who are determined by Adult Protective Services to be victims of abuse, neglect, or exploitation who are in need of immediate services to prevent further harm and are referred by the County, will be given primary consideration for receiving direct services, i.e. Homemaking, Personal Care, etc. As used in this subsection, "primary consideration" means that services must commence within 72 hours after referral to the Contractor. This amendment shall begin on June 15, 2011 or the date on which the amendment has been signed by both parties, whichever is earlier. All provisions in the contract and any attachments thereto in conflict with this amendment shall be and are hereby changed to conform with this amendment. All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the contract. This amendment and all its attachments are hereby made a part of the contract.
