ProposalSECTION 00300 PROPOSAL For PHASE 1 PLANTATION KEY GOVERNMENT CENTER TAX COLLECTOR AND PROPERTY APPRAISER'S OFFICE PLANTATION KEY MONROE COUNTY, FLORIDA Submitted: May 30 , 19 86 Board of County Commissioners Monroe County Courthouse Key West, Florida 33040 Gentlemen: The undersigned, as Bidder, hereby declares that the only person or persons interested in the Proposal, as principal or principals, is or are named herein and that no other person than herein mentioned has any interest in the Proposal of the Contract to which the work pertains; that this Proposal is made without connection or arrangement with any other person, company, or parties making a bid or proposal and that the Proposal is in all respects fair and made in good faith without collusion or fraud. The Bidder further declares that he has examined the site of the work and that from personal knowledge and experience, or that he has made sufficient test holes and/or other subsurface investigations to fully satisfy himself that such site is a correct and suitable one for this work and he assumes full responsibility therefor; that he has examined the Drawings and Specifications for the work and from his own experience or from professional advice that the Drawings and Specifications are sufficient for the work to be done and he has examined the other Contractual Documents relating thereto, including the Advertisement for Bids, Instructions to Bidders, Proposal, Bid Security or Bid Bond, Contract, Performance and Payment Bonds, General and Special Conditions, Technical Specifications, Drawings and has read all addenda prior to the opening of bids, and that he has satisfied himself fully, relative to all matters and conditions with respect to the work to which this Proposal pertains. The Bidder proposes and agrees, if this Proposal is accepted, to contract with Monroe County, Florida, (Owner) in the form of contract specified, to furnish all necessary materials, all equipment, all necessary machinery, tools, apparatus, means of transportation, and labor necessary to complete the work specified in the Proposal and the Contract, and called for by the Drawings and Specifications and in the manner specified. The Bidder further proposes and agrees to comply in all respects with the time limits for commencement and completion of the work as stated in the Contract Form. The Bidder further agrees that the deductions for liquidated damages, as stated in the Contract Form, constitute fixed, agreed, and liquidated damages to reimburse the Owner for additional costs to .the Owner resulting from the work not being completed within the time limit stated in the Contract Form. The Bidder further agrees to execute a Contract and furnish satisfactory Performance 00300 - 1 04-016.82 and Payment Bonds each in the amount of one -hundred percent of the Contract price, within ten consecutive calendar days after written notice being given by the Owner of the award of the Contract, and the undersigned agrees that in case of failure on his part to execute the said Contract and Performance and Payment Bonds within the ten consecutive calendar days after the award of the Contract, the cashier's check or Bid Bond accompanying his bid and the money payable thereon shall be paid to the Owner as liquidation of damages sustained by the Owner; otherwise, the check accompanying the Proposal shall be returned to the undersigned after the Contract is signed and the Performance and Payment Bonds are filed. The undersigned agrees to accept in full compensation therefor the totalof the lump sum prices and extended unit prices items named in the following schedule. - It is understood that the unit prices quoted or established for a particular item are to be used for computing the amount to be paid to the Contractor, based on the quantities actually constructed as determined by the applicable measurement and payment portion of the technical specifications. The bid security shall be in an amount of not less than five percent (5%) of the bid price. 00300 - 2 04-016.82 BID FORM The Contractor shall construct the Phase I - Plantation Key Government Center Tax Collector and Property Appraiser's Office and Supervisor of Elections Enclosures according to the Contract Documents and the original project manual dated March 1986'and Drawings dated December 29, 1985 and Addendum No. 1, dated May 23, 1986 for a Contract Lump Sum amount of Four Hundred and Forty-one Thousand Four Hundred and Sixty-one Dollars ($441,461.00). In order to construct the project stipulated above for this amount, Hewett - Kier Construction, Inc. will incorporate the following deductive and/or substitute items into the completed project: Base Bid (Includes Alt. #8) $357,500.00 1. Alt. # 1 - Complete first floor shell space including, but not limited to partitions, finishes, mechanical, plumbing, and electrical work. + $ 92,000.00 2. Alt. #4 - Add hurricane shutters on all exterior windows. + 3,600.00 3. Alt. # 7 - Provide formica counters in spaces # 1, # 14, # 3 and #13. + 14,200.00 Sub Total $467,300.00 4. Omit permits; water meter connection fees, etc. (to be waived or obtained by Monroe County). - $ 2,200.00 5. Roofing: Remove tapered insulation and replace with lightweight concrete (for sloping) and - maintain regular built-up roof. - 7,500.00 6. Windows: Use local manufactured standard aluminum storefront, triple standard size windows. - 5; 600.00 7. Electrical Service: Provide overhead service to the building in lieu of underground. - 1, 000.00 8. Acoustical Ceilings: Use standard steel T's in lieu of aluminum clad. - 280.00 (Enclosures) - 1, 799.00 (Tax Coll./ Prop. App.) 9. 6" Granite Strip: Eliminate on top of cabinets, substitute formica. - 350.00 (Enclosures) - 1,400.00 (Tax Coll./ Prop. App.) 10. HVAC Balance: To be performed by the Contractor in lieu of independent Engineer. - 600.00 04-016.82/85 00300-3/4 .4 11. Electric Receptacles: Use standard interior in lieu of cast receptacles. 12. Alternate #1: Change oak doors to birch. 13. Enclosures: Construct block stem wall w/cap in lieu of poured concrete. 14. Addendum #1: Use vinyl tile in supervisor's offices in lieu of carpet. 15. Plumbing: All soil pipe to be PVC in lieu of cast iron and toilets w/down discharge and competitive fixtures. 16.
