submitted report on the Card Sound Bridge, completes the required
inspection of the Project.

STRUCTURE LOCATION
This bridge is located 2.494 miles East of the Dade - ;Monroe County Line
and 3.298 miles West of the intersection of State Road S -905 and - 905A
(Card Sound Road). It spans over a channel commonly known as Tubby's
Creek.

GENERAL DESCRIPTION
This structure was designed in accordance with the 1965 Edition of the
AASHO Standard Specifications for Highway Bridges. It was constructed
in accordance with the Florida State Road Department Standard Specific-
tions for Road and Bridge Construction dated 1966 and Special Provision :.
It was designed and constructed for a H2O loading capacity and a >iwc
limit of 60 mph. Construction was completed and the bridgo oponcd to
traffic in July of 1%9.
The Monroe County Beard of County Commissioners has placed a 30,000
•.:c :i ght restriction on the roadway approaches to this bridge a *: c :.i, :
condition of the rrr.d'., - :1y '1r the type of foundation on which F.
constructed. This rest.r:cti on is properly posted.
The bridge is a three spn 100' long, 38' -3" wide cone. 7Lc: :A.. uctur

are overlaid with a mi.r mom o f 1 " Typc I asphaltic,
course. Drainage is aeeompi 1.:3h.t'd with 4" diameter
;3'44" centers on each side of the bridge.
The approaches are protected by standard galvanized guardrail mounted
on precast concrete posts. The roadway is a limerock base with an
asphaltic concrete wearing surface. The driving lanes are 12' each with
one lane eastbound and one lane westbound. The bridge has 14' driving
lanes. The fill for abutments has sand - cement riprap for erosion
protection.
The structure is supported on 18" square prestressed composite concrete
and steel piling. The piling were driven to a minimum of 45 -ton bearing
capacity each with a 10' minimum penetration.
There are four transite telephone ducts encased in concrete located in
the North overhang. This duct bank was installed during construction
of the bridge for future use, but as of this date have rct bccr.
A physical inspection of this structure and an underwater inspection
made to determine existing conditions. A record of the deficiencies.
included as a part of this report.

STRUCTURAL DEFICIENCIES
1. Roadway Approaches: The East approach has a dip about 20' cast of the
bridge. This in turn causes vehicles to impact on the bride. A similar
condition exists on the priest end. There are also cracks in the surface
due to subsiding.
2. Guardrails: Partially obscured by weeds and shrubs.
3. Approach slabs: Expansion joints have opened up due to scttlenmcnt,
and the wing -walls have a 3" to 4." gap.
4. Sidewalks: No deficiencies rioted, except expansion jo
5. Handrails: Spin No.1 has old patches spall.ing off of rail pot.

7. Bridge Drainage: Scuppers were clear. Subsiding at the bridge
allowed storm ester to cause washing.
8. Joints: Due to the continous nature of the wearing surface, the deck
proper has no provision for expansion, in the normal sense of the
word. The deck has a ragged crack over the location of the slab
ends (transverse). Crumbling asphaltic concrete has apcarently entered
the expansion joint voids, opening up the joint with each subsequent
expansion cycle. The overall movement appears to be in both directions
from the center, resulting in 3" to 4" voids at the wing walls.
9. Slope. Protection: West end riprap has a major washout; merits early
repair. East riprap needs chinking. Kill weeds with herbicide.
1p. Abutments: No deficiencies noted.
11. Bent Caps: Cap i#'2 has rebar ends showing on piles #2 and ir'3•
Cap i1 - the patches on the rebar ends on piles #E2 and i,`3 are
spalling off. •
12. Piling: No deficiencies noted except as above.
13. Underwater Inspection: No deficiencies noted. No scouring act.

RECOMMENDED CORRECTIVE ACTION
1. Roadway Approaches: Eliminate dips by overlaying levelling courbes.
Seal other exposed cracks.
2. Guardrails: Cut offending weeds and shrubs.
3. Approach slabs: Clean out joints and reseal. On certain wide joints,
a bulkhead to retain the pour will be needed.
4. Handrails: like repairs to items noted under deficiencies.
5. Decks: Reseal longitudinal cracks. For transverse cracks, se

2. Guardrails: Apply herbicide to obstructing vegetation.
3. Approach slabs: Clean out expansion joints at bridge ends and refill
with suitable expansion joint material. Apply same treatment at wings.
4. Sidewalks: Span No. 1 - South rail has spalls on center post; repa3.:.
Span No. 2 - Repair exposed rebars .
Span No. 3 - Repair e:Tosed rebars.
6. Decks: Seal cracks over longitudinal joints. Saw out and redeveif%;'
transverse sealing.
7. Joints: Remove non - compressible materials in joints of deck,
