At the October 19, 2011, Board of County Commissioner's meeting the Board granted approval and authorized execution of the following: Item C45 First Amendment to Agreement with Pedro Falcon Electrical Contractor's Inc. for the Renovation and Addition of Fire Station # 17 in Conch Key. Item C46 Contract with Currie, Sowards, Aguila Architects, (CSA), to draw a preliminary / design of a parking facility on Thomas Street. ✓ Item C47 Contract for Professional Services with William P. Horn Architect P.A. for Restroom and Shower Facilities at the Crawl Key Fire Training site. Item C52 Task Order Contract with MBI /K2M Architects under their continuing services agreement with the County. This task order contract for NTE $7,000 will provide initial site layout and feasibility grade cost estimate to construct a Customs and Border Protection (CBP) area at the Marathon Airport in the area identified by CBP representatives. Should you have any questions, please do not hesitate to contact this office. cc: County Attorney Finance File K CONTRACT FOR PROFESSIONAL SERVICES FOR THE CRAWL KEY FIRE TRAINING FACILITY In accordaase with the Continuing Contract for Professional Services made and entered on the 16 day of December 2009 between Monroe County ( "Owner" or "County") and William P. Horn, Architect, P-A, CArchitect") where pursuant to Florida Statute 287.055 (2) (g) Professional Services are to be rendered for projects in which the estimated construction costs of each individual project under the contract does not exceed Two Million dollars ($2,000,000.00). This contract shall be effective on the 19* day of October, 2011. All terms and conditions of the Continuing Contract for Professional Services, attached hereto as `Exhibit A ", shall apply to this contract unless expressly modified in this contract. The Architect warrants that he has not employed or retained any company or person, other than a bona fide employee working solely for the Architect to solicit or secure this agreement and that he has not paid or agreed to pay any person, company, corporation, individual, or firm, other than a bona fide employee working solely for the Architect any fee, commission, percentage, gift, or other consideration contingent upon or resulting from the award or making of this agreement. SCOPE OF WORK ArchitecturaVEngineermg services for the new Crawl Key Fire Training Facility to include a new 700 square foot one story building that will include two toilet rooms, six showers, lockers, and changing areas. The building will have to be raised 3-4 feet for flooding, and a ramp can be added on the outside for access. The building will be a prefabricated building. Onsite work will consist of concrete piles and piers for the foundation and a wooden handicap ramp for access. DESIGN PHASE The Architect shall design the building and location on the site (create a site plan, design floor plan, and elevations). A schematic design must be approved in writing by the Owner prior to Architect continuing to the Design Development/Document Phase. DESIGN DEVELOPMENT /DOCUMENT PHASE The Architect shall provide drawings for the foundation and ramp and develop design specifications for the prefabricated unit. Drawings and specifications must be approved in writing by the Owner prior to =Arcing to the Bidding Phase. The Architect shall provide an estimate of anticipated costs. BIDDING PHASE The architect shall hold a pre -bid conference at the site, review bids, and make a recommendation. CONSTRUCTION ADMEMTRATION PHASE Architect shall make two site visits for inspections during construction, and review shop drawings. PAYMENT Terms of payment will be a total contract dollar amount as listed below in three Phases. Details of payment are as specified below. The Architect shall submit monthly, unless otherwise agreed in writing by the Owner, and invoice to the Owner requesting payment for services properly rendered and reimbursable expenses due hereunder and shall indicate whether the work completed applies to the (1) Design, Construction Documents, and Specifications Phase, (2) the Bidding Phase for the contractor, or the (3) the Construction Administration Phase. The Architect's invoice shall describe with reasonable particularly the services rendered. The Architect's invoice shall be accompanied by such documentation or data as required by the Owner to support of expenses for which payment is sought.
