Enclosed, are two duplicate originals of the subject Amendment, executed on behalf of Monroe County. It is my understanding that your office will follow through on the above with the Alliance for Aging, Inc. Please be sure that one fully executed copy is returned to this office as soon as possible. Should you have any questions concerning the above, please do not hesitate to contact me. cc: County Attorney Finance County Administrator, w/o document File Amendment #1 AMENDMENT TO 1994 -95 COMMUNITY CARE FOR THE ELDERLY CONTRACT THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred to as the "Alliance ", and Monroe County Board of Commissioners hereinafter referred to as the "Provider ", amends contract KG -451E. 1. Section II, Paragraph A, Contract Amount, is hereby amended to read: Subject to the availability of funds, the Alliance will reimburse no more than a total dollar amount of $527,987.00 for expenditures made in accordance with the approved budget for Community Care for the Elderly funds. The Alliance's performance and obligation to pay under this contract is contingent upon an annual appropriation by the State of Florida Department of Elder Affairs. The costs of services paid under any other contract or from any other source are not eligible for reimbursement under this contract. 2. Section III, Paragraph G -1, Match, is hereby amended to read: The total match for this contract period will be at least $58,665.00. The provider's match will be made in the form of cash and /or inkind resources. At the end of the contract period, all Community Care for the Elderly funds expended must be properly matched. 3. Attachment 1, Summary Budget, is hereby amended and attached hereto. 4. This amendment shall begin on September 30, 1994 or on the date on which the amendment has been signed by both parties, whichever is later. All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the contract. , This amendment and all its attachments are hereby made a part of the contract.
