Amendment 001 Amendment 001 Rate Agreement US -551 Page 1 THIS AMENDMENT entered into between the Alliance for Aging, Inc., hereinafter referred to as the "Alliance ", and the Monroe County Board of Commissioners hereinafter referred to as the "provider ", amends Rate Agreement US -551. The purpose of this amendment is to change the rate of reimbursement for eligible congregate and home delivered meals from $0.6146 to $0.60 as authorized by the USDA /FNS Grant Award Reference Number F53615 for meals provided from October 1, 1994 through September 30, 1995. This rate decrease is retroactive to October 1, 1994. The period of the rate agreement is extended for an additional six months to September 30, 1996 to allow for any additional rate adjustments. 1. Section II, Paragraph A, Contract Amount, is hereby amended to read as follows: A. To make payments for services identified in Section I.A. of this rate agreement at the rates stipulated below, in w ail amount not to exceed $ 32,481.60, subject to the c a`t�ailability of funds. Section III, Paragraph B, Ending Date, is hereby amended to read as follows: B. This rate agreement shall end on September 30, 1996. 3. Section III, Paragraph C, Method of Payment, Number 5 is hereby amended to read: This rate agreement is for services provided during the 1995 Federal Fiscal year beginning October 1, 1994 through September 30, 1995. The additional twelve months (October 1, 1995 through September 30, 1996) are to allow rates to be adjusted for the twelve month service period. Rate adjustments will be based on the final reimbursement rate established by the USDA. This rate agreement shall automatically terminate after the final rate for the federal fiscal year has been established and the release of final payments are authorized by the department. 5. This amendment shall begin on May 5, 1995 or the date on which the amendment is signed by both parties, whichever is later. All provisions in the rate agreement and any attachments thereto in conflict with this amendment shall be and are hereby changed to conform with this amendment. All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the rate agreement. This amendment and all its attachments are hereby made a part of the rate agreement.
