On April 17, 1996, the Board of County Commissioners granted approval and
authorized execution of the following:
Amendment No. 2 to Contract No. KG-551E, between Monroe County and the
Alliance for Aging, Inc.
Amendment No. 1 to Contract No. KG -007 between Monroe County /Monroe
County In Home Services and Florida Department of Health and
Rehabilitative Services.
Enclosed please find two duplicate originals of each contract, executed on behalf
of Monroe County. Please be sure that a fully executed copy of each is returned to this
office as soon as possible.

Amendment #2
AMENDMENT TO 1995 -96
COMMUNITY CARE FOR THE ELDERLY CONTRACT
(ADVANCE /COST REIMBURSEMENT)
THIS AMENDMENT, entered into between the Alliance for Aging,
Inc. hereinafter referred to as the "Alliance ", and the
Monroe County Board of Commissioners hereinafter referred to
as the "Provider ", amends contract KG -551E.
The purpose of this amendment is to increase the contract
amount by $5,000.00 in order to purchase electronic data
equipment.
1. Section II, Paragraph A, Contract Amount, is hereby
amended to read:
Subject to the availability of funds, the Alliance will
reimburse no more than a total dollar amount of
$549,221.00 for expenditures made in accordance with
the approved budget for Community Care for the Elderly
funds, ATTACHMENT II to this contract. The Alliance's
performance and obligation to pay under this contract
is contingent upon an annual appropriation by the
Department of Elder Affairs. The costs of services
paid under any other contract or from any other source
are not eligible for reimbursement under this contract.
2. Section III, Paragraph G -1, Match, is hereby amended to
read:
The total match for this contract period will be at
least $61,025.00. The provider's match will be made in
the form of cash and /or in -kind resources. At the end
of the contract period, all Comunity Care for the
Elderly funds must be properly matched.
3. Attachment II, Summary Budget, is hereby amended and
attached hereto.
4. This amendment shall begin on March 31, 1996 or on the
date on which the amendment has been signed by both
parties, whichever is later.

All provisions in the contract and any attachments thereto
in conflict with this amendment shall be and are hereby
changed to conform with this amendment.
All provisions not in conflict with this amendment are still
in effect and are to be performed at the level specified in
the contract.
This amendment and all its attachments are hereby made a
part of the contract.
IN WITNESS WHEREOF, the parties hereto have caused this 3
page amendment to be executed by their undersigned officials
as duly authorized.
