At the April 16, 1997 County Commission Meeting, the Board granted approval
and authorized execution of Amendment #2 to the Community Care for the Elderly
Contract, No. KG-651E between Monroe County and The Alliance for Aging, Inc.
Enclosed please find two duplicate originals of the above Amendment, executed
on behalf of Monroe County. Please be sure that one fully executed copy is returned to
this office as soon as possible.

IMMEDIATE PAST PRESIDE Waiver clause in your contract to make it conform with the same change

Amendment #2
AMENDMENT TO 1996 -97
COMMUNITY CARE FOR THE ELDERLY CONTRACT
(ADVANCE /COST REIMBURSEMENT)
THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance ", and the Monroe County Board of Commissioners hereinafter referred to
as the "Provider ", amends contract KG -651E.
The purpose of this amendment is to change the Medicaid Waiver dause.
1. Section III, Paragraph G.15, is hereby amended to read:
The provider will, through the assessment by case managers, identify Medicaid
eligible CCE clients who meet Medicaid Waiver criteria and refer them to CARES
and SSI related Payments (if applicable) for approval for Medicaid Waiver services.
Approximately 44% of the general revenue funds necessary to support approved
clients will be transferred to match the 56% federal funds in order to increase
provider Medicaid Waiver spending authority to serve the clients. The identified
funding for this transfer activity will be reported to the Alliance for submission to
the department by October 1 of the fiscal year.
4. This amendment shall begin on April 15, 1997 or on the date on which the
amendment has been signed by both parties, whichever is later.
All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed
at the level specified in the contract.
This amendment and all its attachments are hereby made a part of the contract.
IN WITNESS WHEREOF, the parties hereto have caused this amendment to be executed by
their undersigned officials as duly authorized.
