At the May 20, 1997 BOCC meeting, the Board granted approval and authorized
execution of Amendment #2 to the 1996 -97 Alzheimer Disease Initiative Contract
(Advanced/Cost Reimbursement).
Attached hereto are two duplicate originals of the Amendment for your follow - through
with the Alliance. Please be sure that one fully executed copy is returned to this office as
quickly as possible.

AMENDMENT TO 1996 -97
ALZHEIMER DISEASE INITIATIVE CONTRACT
(ADVANCE /COST REIMBURSEMENT)
THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter
referred to as the "Alliance ", and the Monroe County Board of Commissioners
hereinafter referred to as the Provider", amends contract KG -697.
The purpose of this amendment is to decrease the contract amount by a projected
surplus of $13,000.00 in order to redistribute it among other ADI providers able to
spend it by the end of the contract period.
1. Section 11, Paragraph A, Contract Amount, is hereby amended to read:
Subject to the availability of funds, the Alliance will reimburse no more than a
total dollar amount of $42,458.00 for expenditures made in accordance with
the approved budget for Alzheimer Disease Initiative funds. The Alliance's
obligation to pay under this contract is contingent upon an annual appropriation
by the State of Florida Department of Elder Affairs. The costs of services paid
under any other contract or from any other source are not eligible for
reimbursement under this contract.
2. Attachment 1l, Summary Budget, is hereby amended and attached hereto.
4. This amendment shall begin on June 10, 1997 or on the date on which the
amendment has been signed by both parties, whichever is later.
All provisions in the contract and any attachments thereto in conflict with this
amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be
performed at the level specified in the contract.
This amendment and all its attachments are hereby made a part of the contract.
