Attached please find four duplicate originals of the above Agreement, executed on behalf of Monroe County. Please be sure that one fully executed copy is returned to this office as soon as possible. If you have any questions concerning the above, please do not hesitate to contact me. cc: County Attorney Finance County Administrator, w/o document File • CON I KAL I: 92SLA -A-1 i LIHEAP MODIFICATION NO: 001 MODIFICATION OF AGREEMENT BETWEEN FLORIDA DEPARTMENT OF COMMUNITY AFFAIRS AND Monroe County Board of County Commissioners The agreement for the period April 1, 1998 to March 31, 1999,, between the Department of Community Affairs and Monroe County Board of County Commissioners, bearing the above contract number is modified by mutual agreement of the parties as follows: Clause (16)(a), FUNDING /CONSIDERATION, is hereby amended to read: "This is a cost reimbursement agreement. The Recipient shall be reimbursed for costs incurred in the satisfactory performance of work hereunder in an amount not to exceed $91,335, subject to the availability of cash." This amount includes: 1. $ 77,677 Current FFY 1998 -99 contract allocation. 2. $ 13,658 Additional funding available to your agency. • Attachment B, Section D, (1): OTHER PROVISIONS, is hereby amended to read: "(1) The Recipient must budget between six (6) and twenty -five (25) percent of the total Agreement amount for crisis assistance. Two (2) percent of the total Agreement funds must be held in the weather related or supply shortage program category until December 15 of each program year for a possible emergency or crisis situation. A weather related or supply shortage crisis program may be instituted upon declaration or order by the Department, Governor or President that a crisis or emergency exists due to weather related conditions or a supply shortage that would adversely affect low income households. If the Recipient or the Department finds that the two percent will not be a sufficient amount to meet the emergency or crisis, the Recipient may, if needed to meet the emergency or crisis, draw on other Agreement categories up to twenty aercent (20 %) of the total Agreement without additional written authorization. After December 15, if no such emergency or crisis has been declared or ordered, the Recipient will allocate these funds to the crisis or the home energy category of the program without additional written authorization from the Department." ATTACHMENT A: Budget and Scope of Work: Budget Changes Work Plan Changes All other provisions of the contract not in conflict with this modification remain in full force and effect.
