On April 21, 1994 the Board of County Commissioners granted approval and
authorized execution of an Agreement between Monroe County and the Florida
Department of Community Affairs concerning Emergency Management Base Grant (9G-
19 Rule), Contract No. 94- EO -4M -11 -54-01 -044.
Attached for your follow- through with DCA are four executed duplicate originals.
Three for return to DCA per their instructions, and one for your files. Please be sure
that one fully executed original is returned to this office as soon as possible.

Dear Mr. Wagner:
Enclosed is a fully executed copy of the 1993 -94 Emergency
Management Preparedness and Assistance Program Base Grant
Agreement between your county and the Department of Community
Affairs. Also included is a copy of (1) the Financial
Report /Reimbursement Request forms to be used when filing a claim
for reimbursement and to be submitted quarterly as a financial
status report; and (2) the Summary Project Progress Report which
is also required for quarterly reporting to this Division on the
program.
If you have requested an advance of funds under Section B,
Attachment F of this Agreement, but have not provided us with a
written letter of justification explaining the need for and the
anticipated use of any advanced funds, please do so as soon as
possible. Absence of this information could cause a delay in the
release of advanced funds to you.
Please provide the Division's Grants /Fiscal Administration
Section with a copy of the Position Description for the staff
position identified with this program as defined in
Section 9G- 19.002 (6) or (8), Florida Administrative Code and
certified by your county in Section XVI of this Agreement.
