Pursuant to an Interagency Agreement between the United States Departmentsof Labor, Agriculture, and Interior,.the United States Departments of Agriculture and Interior local YACC project directors will place job orders with the FSES office; 2) will make selections from referrals from the FSES office; 3) will notify candidates of selection; 4) will verify employment. . • A. The Prime Sponsor or Participating Agency Agrees to: • 1. -Refer youth to the FSES with an approved referral form. B. The Florida State Employment Service agrees to: 1. Partially register the applicant, complete the YACC application, and identify the recruiting agency =There applicable. 2. Provide general information on the 'ACC program and• to advise candidates: (a) that permission. must be obtained to release ES -511 and YACC form (signature required on the YACC form); (b) that if they are selected they will receive information from the YACC project or camp director on the type of work to which they will be assigned, and the date, time, and place they are to report to work; (c) that if fhey receive a job offer, they will also receive a physical examination form from the camp or project director that must be completed and presented to'the camp or project director on the day they report for work; (d) that if they are selected for jobs at residential camps and are under the age of majority they may also receive parental consent form, which must be signed by their parent(s) or legal guardian and presented to the director on arrival at the camp; and (e) about where the candidate may obtain a free or low -cost physical examination if they are unable to afford or pay for one. (This advice might be where to obtain .a medicaid card or the address of a free clinic.) 3. .Forward to the YACC project director or appropriate USDA or USDI . field representative a copy of the ES -511 along with the stapled YACC form for each referral. • 4.
