At the March 21, 2012, Board of County Commissioner's meeting, the Board granted
approval and authorized execution of the following:

janitorial services at the George Dolezal Marathon Library.

AMENDMENT AND RENWAL AGREEMENT FOR JANITORIAL SERVICES
MAY HILL RUSSELL LIBRARY AND/OR
DEPARTMENT OF JUVENILE JUSTICE BUILDING - SECOND FLOOR
his MENDMENT AND RENEWAL AGREEMENT is entered into this Jyoe�—day of
, 2012, between MONROE COUNTY, FLORIDA ("COUNTY"), a political subdivision of
the State of Florida, whose address is 1100 Simonton Street, Key West, Florida 33040, and DIVERSIFIED
SERVICES OF KEY WEST, INC. ("CONTRACTOR"), a Florida corporation, whose address is 50-D 11
Avenue, Key West, Florida 33040.
WHEREAS, on June 15, 2011, the parties entered into an agreement to provide janitorial services for
the May Hill Russell Library and/or the Department of Juvenile Justice Building — Second Floor, Monroe
County (hereinafter "Original Agreement"); and
WHEREAS, on September 15, 2011, the County extended the number of days the library is to be open;
and
WHEREAS, the parties hereto wish to amend the contract to add one additional day per week of
janitorial services at the library which will increase the monthly fee; and
WHEREAS, in accordance with the terms of the original contract, the parties wish to exercise the first
renewal option, extending the contract term for one additional year and increasing the monthly contract amount
for the renewal term by 3% CPI-U;
NOW, THEREFORE, in consideration of the mutual promises and covenants set forth below, the
parties agree to amend the original contract dated June 15, 2011, as follows:
1. Item 2 Scope of the Work, subparagraph G shall read as follows:
Janitorial services will be six (6) days a week, on the days the library is open (currently Monday
through Saturday, excluding holidays), beginning at 7:00 a.m., and finishing no later than the Library
opening time of 9:30 a.m. Should the Contractor need additional time to complete the scope of work, a
mutually agreed upon schedule between the Contractor and the Library allowing the Contractor to start
prior to 7:00 a.m. may be implemented.
2. Item 2 Scope of the Work, subparagraph H shall read as follows:
The Contractor shall be responsible to coordinate the work with the County Lower Keys Facilities
Maintenance Office, contact phone (305) 292-4431 or (305) 292-4436, Monday through Friday, 8:00 to
5:00 pm. The Contractor shall perform the work during hours and time as specified.
3. Item 3 Payments to Contractor, subparagraph D shall read as follows:

From April 1, 2012 through June 30, 2012: ONE THOUSAND FIVE HUNDRED EIGHTY AND
NO/100 DOLLARS ($1,580.00)
From July 1, 2012 through June 30, 2013: ONE THOUSAND SIX HUNDRED TWENTY
SEVEN AND 40/100 DOLLARS ($1,627.40)
4. Item 3 Payments to Contractor, subparagraph E shall read as follows:

BUILDING — SECOND FLOOR:
From July 1, 2011 through June 30, 2012: TWO THOUSAND SIX HUNDRED AND NO/100
DOLLARS ($2,600.00)
From July 1, 2012 through June 30, 2013: TWO THOUSAND SIX HUNDRED SEVENTY
EIGHT AND NO/100 DOLLARS ($2,678.00).
5. Item 4 Term of A reement shall read as follows:
This Agreement shall commence July 1, 2011 and end on June 30, 2013, unless terminated earlier
under paragraph 18 of this Agreement.
The County shall have the option to renew this Agreement for one (1) additional one- year period at
the terms and conditions mutually agreeable to the parties, exercisable upon written notice given at least 30
days prior to the end of the renewal term.
The Contract amount shall be adjusted annual in accordance with the percentage change in the U.S.
Department of Commerce Consumer Price Index (CPI-U) for all Urban Consumers as reported by the U.S.
Bureau of Labor Statistics at December 31 of the previous year using the most recently published indicator.
6. Except as set forth above, in all other respects, the terms and conditions set forth in the Original Agreement
remain in full force and effect.
