Ocean Reef. The task order is being awarded under the On Call Professional Engineering
Services contract with EAC Consulting, Inc. CR 905 design item.

TASK ORDER FOR ON CALL PROFESSIONAL SERVICES BETWEEN
MONROE COUNTY AND EAC CONSULTING, INC.
FOR
CR 905 BIKE LANES PROJECT DESIGN
In accordance with the Continuing Contract for On Call Professional Engineering
Services made and entered on the 20th day of January 2010 between Monroe County
hereinafter referred to as the "County" and EAC Consulting, Inc. hereinafter referred to
as "Consultant" where design services are allowed if construction costs do not exceed
$2,000,000.
All terms and conditions of the referenced Contract For On Call Professional
Engineering Services apply to the Task Order, unless the Task Order modifies an Article
of the Agreement of which will be specifically referenced in this Task Order and the
modification shall be precisely described.
This Task Order is effective on the 18th Day of April, 2012.
Article II Scope of Basic Services, Paragraph 2.1.1 is amended as follows:
The scope of services for the County Road (CR) 905 Bike Lanes project will include
completion of design for construction and any required permitting to construct five foot
wide (minimum) bike lanes on CR 905 from US 1 in Key Largo to Ocean Reef in North
Key Largo (approximately 10.5 miles).
The Design for Construction shall include, but shall not necessarily be limited to, plans
and specifications which describe all systems, elements, details, components, materials,
equipment, and other information necessary for construction. The Design for
Construction shall be accurate, coordinated and in all respects adequate for construction
and shall be in conformity, and comply, with all applicable law, codes, permits, and
regulations. Products, equipment and materials specified for use shall be readily
available unless written authorization to the contrary is given by the County.
Details of the scope of services are outlined below and included in the Consultant's
proposal that is attached to this task order as Exhibit A.
1.0 DESIGN DEVELOPMENT
The Consultant will evaluate existing adjacent environment and habitat and provide a
design that eliminates or minimizes impacts to the surrounding environment and habitat.
2.0 CONSTRUCTION DOCUMENTS PHASE
2.1 The Consultant shall prepare, for approval by the County, Construction
Documents consisting of Drawings and Specifications setting forth in detail the
requirements for the construction of the project. Construction documents shall conform
to the standards contained in the following:

2.2. The Consultant shall provide Drawings and applicable Technical for the County's
review.
2.3 Upon completion of the Construction Documents Phase, the Consultant shall
provide Construction Documents for the County's approval. Upon approval by the
County the Consultant shall provide the County up to 5 sets of Construction Documents
that have been signed and sealed by the Engineer. The Consultant shall also provide
an electronic version of the construction documents. The Consultant shall provide an
estimate of anticipated construction cost in accordance with the construction
development phase.
2.4 The Consultant shall assist the County in the preparation of the necessary
bidding information for the production of bidding forms, the Conditions of the Contracts,
and the forms of Agreements between the County and the Contractors by providing
supporting information as to the projects scope, bid items, estimated quantities and
construction duration. The County shall prepare all Bidding Forms, Conditions of the
Contract, and Forms of Agreement.
2.5 The Consultant's construction documents (plans, specifications, etc) will conform
to all codes and regulations of the federal government, county, state, municipalities,
agencies and state departments, in effect at the date of this Agreement, and shall be of
such completion as to be acceptable for review and ruling by said agencies when
permits are applied for. The Consultant shall use due care in determining permit
requirements and shall meet with regulatory agencies as necessary to coordinate

specific permit requirements. The Consultant shall document all meetings and
conversations with said regulatory agencies. If permits are denied for incompleteness or
for lack of following said codes or regulations, or permit requirements, then the Engineer
will conform the construction documents in such manner to receive permits upon such
plans. Work required by the Consultant to conform documents to federal, state, city,
county, or agency specifications to allow them to be approved shall be completed at no
charge or cost to the County, unless said requirements are changed during the course of
the project.
2.6 The County shall be responsible for the timely submittal of all permit application
fees.
2.7 At the 30% and 100% design phases the Consultant shall provide drawings and
other documents which depict the current status of design for the County's review and
information. The Consultant shall provide an estimate of anticipated construction costs
and construction schedule.
2.8 As needed, the Engineer will provide clarification and answers to questions from
prospective bidders during the construction bid process. Answers will be provided in a
timely manner in order to facilitate bidding.
3.0 CONSTRUCTION DOCUMENTS PHASE REQUIREMENTS
To satisfactorily perform the Construction Documents phase requirement, the
Engineer must complete the tasks set forth in items 3.1 through 3.4.
3.1 Construction Plans — This consists of, at a minimum, Key Sheet, Summary of
Pay Items and Quantities, Drainage Map, Project Layout, Plan and Profile sheets,
Typical Sections, Detail sheets, General Notes, Traffic Control Plan. Construction plans
shall be in accordance with FDOT Plans Preparation Manual.
3.2 Specifications — For general specifications, FDOT Specifications will be used.
Comprehensive, abbreviated methods, materials and systems descriptions in tune with
the drawings will be developed as necessary with Technical Special Provisions.
3.3 Schedules — Prepare an estimate of the Construction Time.
3.4 Estimate of Construction Cost — Estimate of anticipated cost in accordance
with the Construction Documents.
4.0 CONSTRUCTION COST
Contemporaneously with the submission of the Design, the Consultant shall submit to
the County in writing its final estimate of the contractor's anticipated bid price for
constructing the Project. Once submitted, the final anticipated price estimate shall be
adjusted by the Consultant to reflect any increase or decrease in anticipated price
resulting from a change in Design.
4.1 The Construction Cost shall be the total estimated bid cost to the County of all
elements of the Project designed or specified by the Engineer.

4.2 The Construction Cost shall include the cost at current market rates of labor and
materials and Equipment designed, specified, selected or specially provided for by the
Engineer, plus a reasonable allowance for Contractor's overhead and profit.
4.3 Construction cost does not include the compensation of the Consultant and the
sub - consultants, the costs of land, rights -of -way, financing or other costs which are the
responsibility of the County.
4.4 The Engineer agrees that, should the bid for construction of the project exceed
its estimate by ten percent (10 %) or more, it will redesign, redraw or rebid, at no
additional cost or expense to the County, until the bids are within the stated limits.
Construction Phase services will be included in future task orders.
Article VII, Paragraph 7.1 is amended to include the following:

ATTACHMENT 1
SCOPE OF SERVICES
Background
Monroe County has requested that EAC Consulting, Inc. (EAC)., provide a fee proposal to undertake requisite
professional services related to the design of five -foot (6) wide (min.) bicycle lanes along CR 905 from Ocean Reef,
south to the intersection with US 1 (approximately 10.5 mi.) in Key Largo, Monroe County, Florida.
Accordingly, the following project tasks are required to undertake the requisite professional engineering services for
the project are described as follows:
TASK 1: PRE -DSIGN ENGINEERING SERVICES, GEOTECHNICAL EXPLORATION AND TOPOGRAOHIC
SURVEY
Task 1.1— Project Management
EAC will provide Project Management services that will comprise of contract administration, preparation of invoices,
coordination of project staff and other sub consultants work, monitoring the project's progress and project costs
during the project's duration.
EAC will coordinate a project kick -off meeting with sub consultants after receiving a notice -to- proceed for this project.
The purpose of this meeting will be to initiate the project, which includes identifying project protocols, establishing
coordination between EAC, and sub - consultants, as well as to request and collect all available data and documents
from all available sources regarding the CR 905 ROW /Roadway.
Task 1.2 — Meetings
EAC will attend a maximum of 4 Coordination Meetings with County agencies or departments and permitting and
regulatory agencies to discuss and resolve pertinent project related issues.
Task 1.3 — Field / Site Visits
Subsequent to the project kick -off meeting, EAC will schedule one additional field site visits as follows:
With the project Team to discuss, review and familiarize the team with project conditions, identify
coordination items and issues as well as developing a project implementation plan that addresses all data
collection and engineering design needs of the project. In addition, this field visit should serve to facilitate
the design team's familiarization and understanding of any unique characteristics that may exist.
Task 1.4 — Topographical Survey and Mapping
These services will be provided by EAC's sub consultant F.R Aleman & Associates, Inc. (FRA). These services will
comprise the undertaking of a full topographic and boundary survey that meets Minimum Technical Standards according
to the Florida Administrative Code (FAC). Survey will provide specific information required to undertake the requisite
professional engineering analyses and design services.
Upon the substantial completion of this task, the developed survey will be provided as a deliverable to the County for
review purposes. Upon the review, the County shall provide comments as necessary which may be incorporated as
updates to the developed survey.
Task 1.5 — Geotechnical / Soil Investigations
These services will be provided by EAC's sub consultant HR Engineering Services, Inc. (HRE). Services will include a
detailed and certified geotechnical report that includes specific information required to undertake the requisite
professional engineering analyses and design services. The report must document the suitability of the proposed site for
proper storm water run -off dissipation. The report will contain at a minimum; SPT Borings (8 feet deep), soil
classification, soil stratification, soil testing to ensure that the proposed site is free from aggressive soils, plastic soils and
chemicals, a determination of measured groundwater levels, normal seasonal high groundwater levels, etc. The SPT
borings shall be located as follows: 1) from Ocean Reef to approximately 1 mile south of the intersection with Card
Sound Road (2.6 miles = 15 borings), the borings shall per performed at 1,000' intervals (staggered about the roadway),
2) from a point approximately 1 mile south of the intersection with Card Sound Road to a point approximately 1.9 miles
north of the intersection with US 1 (approx.. 6.15 miles = 34 borings), the borings shall be performed at 1,000' intervals
(staggered about the roadway), and 3) for the final 1.9 miles (From the intersection with US 1 north), no borings shall be
performed under this contract. It is understood that the County shall provide boring test results for this segment.
The geotechnical scope of work will also include 10 soil percolation tests, and 10 double ring infiltrometer tests for
determining the surrounding soil's ability to absorb storm water runoff. This will be used to assess drainage impacts due
to the addition of impervious surfaces (bicycle lanes).
Upon the substantial completion of this task, the developed geotechnical investigation report (and recommendations) will
be provided as a deliverable to the County for review purposes. Upon the review, the County shall provide comments as
necessary which may be incorporated as updates to the report.
Task 1.6 — Utility Coordination
EAC shall contact all existing utility owners, as identified by Sunshine State One Call of Florida (SSOCOF) based on the
project limits. EAC will request copies of as -built information related to facilities located within the project limits.
Task 1.7 — Preliminary Environmental Assessment
EAC, through its sub consultant Consulting Engineering & Science, Inc., shall conduct a field review of the project
limits to verify the field survey and topography for potential wetland impacts. This task only includes the delineation of
wetlands to be impacted by the proposed bike lanes. This review shall be coordinated with USACE and SFWMD. This
task will provide EAC with a summary of findings, documenting observations and potential concerns that must be
considered during engineering design.
Task 1.8 — Data Collection & Review
EAC shall coordinate with applicable Monroe County departments and divisions to obtain all records of infrastructure
works including but not limited to existing plans, As- Builts, record drawings, studies, reports and any pertinent data that
may impact or facilitate the proposed design services. This may include, but not be limited to roadways, drainage
systems, traffic signals, etc.
TASK 2: DESIGN DEVELOPMENT (30% PHASE)
This work will include the engineering design analyses and preliminary plans necessary for the proposed bicycle lanes
along CR 905. All design work shall be in compliance with the Florida Department of Transportation (FDOT) Standards,
MUTCD, Florida Green book, Monroe County Engineering Standards (where applicable and available), Florida
Administrative Code and all other requirements identified in the Task Order.
It is anticipated that the County will provide EAC with one set of consolidated comments regarding the 30% design
development documents. EAC will schedule a comment resolution meeting with the County, as necessary, to ensure that
the intent of the comments is clearly understood. It is anticipated that prior to the commencement of the 100%
Deliverable Phase, all comments are resolved for incorporation into the final construction documents.
EAC will prepare plans, profiles, cross sections, details and notes for the project including: Summary of Pay Items,
Drainage Structures (as applicable), project signage and striping.
The 30% Design Development Plans will include:
• Key Sheet
• General Information Plans
• Horizontal and Vertical Control Plan Sheet(s)
• Project Layout Plans
• Preliminary Drainage Map
• Typical Section Sheet(s)
• Preliminary Roadway Plans (with Profile Grid)
• Preliminary Traffic Control Details
EAC will provide a preliminary Engineer's Estimate of Probable Cost of Construction at this stage
This fee proposal assumes that the wetlands assessment/delineation for potential impacts (to be performed by our
sub - consultant - CES) will occur after the topographic survey has been completed and at the end of the 30% design
Phase (prior to the 100% Phase). This activity shall be closely coordinated with USACE and SFWMD.
TASK 3 — CONSTRUCTION DOCUMENTS SERVICES (100% PHASE)
This work will include the final incorporation of all design review comments and the preparation of complete construction
documents to a 100% level of completion. EAC will finalize all plans, profiles, cross sections, details and notes for the
project including: Summary of Pay Items, Engineer's Estimate of Probable Cost and Technical Specifications.
In addition, EAC will submit a final Engineer's Report documenting the engineering analyses considerations, approach,
design basis, a feasibility assessment and recommendations for construction improvements.
The 100% Construction Plans will include:

• General Information Plans
• Summary of Pay Items and Quantities
• Drainage Map
• Summary of Drainage Structures (if applicable)
• Horizontal and Vertical Control Plan Sheet(s)

TASK 4 — CIVIL ENGINEERING PERMITTING
Task 4.1 — Environmental Permitting /Approvals
The permitting and review /approvals for this project require in -depth assessment prior to application. As such, EAC
shall research, via field review, for endangered species and essential fish habitats that may be affected by the
design. This will be coordinated with USACE, USFWS and NMFS.
EAC will prepare the ERP and USACE Dredge and Fill Permit only if it is determined that it is necessary based on
whether there are any wetland impacts. The assumption is that these will not be individual permits and will not
require substantial permit preparation, responses and coordination with the regulatory agencies.
OPTIONAL SERVICES
OS 1— Bid and Award Support Services
Under this optional service, EAC would assist the County during the bidding phase by performing the following services:
• Preparation Bid Package and Review of Front -end documents.
• Attendance at Meetings with County
• Attend One (1) Pre -Bid Meeting
• Perform One (1) Project Walk Through with interested Bidders
• Assist the County in the preparation of Addenda as necessary.
• Review submitted bids for concurrence and acceptability
• Verify References submitted by Low bidder for acceptability
• Prepare and Issue Concurrence Memorandum substantiating evaluation of low bidders bid
OS 2 — Limited Construction Administration Support Services
Under this optional service, EAC would perform construction administration oversight support and specialty inspections
services. This includes:
• Attend one (1) Pre - Construction Meeting
• Preparation of Pre Construction Meeting Minutes.
• Respond to RFI and change order requests
• Issue design clarifications
• Review contractor submittal (schedules, samples, materials, shop /installation drawings, etc)
• Participate in specialty inspections & prepare the punch list for substantial completion and final acceptance.
• Shop Drawings Review and Approval
• Observation Services — Bi- monthly specialty site visits and reporting of construction progress.
• Attendance at major Construction Progress Meetings
• Attend the testing /inspection of constructed work with jurisdictional and testing agencies as scheduled by
Contractor.
• Perform final walk- through and prepare punch -list
OS 3 — Resident Project representative (RPR) Services
Under this optional service, EAC Consulting, Inc., would, at the discretion of the County, provide Full Time resident
project representative /inspection services. For the purpose of scheduling, it is anticipated that the construction duration
will be approximately three months. The full time Resident Project Representative (RPR) will act on the County's behalf
to manage the construction operations on a daily basis. In addition, the RPR's responsibilities shall primarily include the
following;
• Attend meetings, both formal and informal, between the EAC team, County staff and the successful
contractor. These meetings will occur informally on a weekly basis, and formally on a biweekly basis. They
will serve as forums to discuss any schedule and or cost concerns, as to resolve problems before they
become critical. The contractor will be required to submit a detailed schedule to the RPR at the pre -
construction meeting. This schedule will be reviewed and approved by the RPR and Monroe County
Engineering. This schedule will be updated in by weekly basis. The RPR will be responsible for reviewing
the contactor's schedule to confirm accuracy of the work activities completed. Analysis of the contractor
schedule will be on the basis of planned versus actual costs for the month and contract to date. A
consolidated contact progress report will be developed
• The RPR will continuously review the contactor's performance, report status, check for variance or delays
and recommend work around and adjustment that can be initiated to get the contractor back on schedule
and or within budget. Quality control during construction will be the responsibility of the contractor. EAC's
staff will support the County by requiring the contractor to develop and submit an acceptable quality control
program prior to commencement of construction. This program will describe, in detail, the contactor's
quality control, organization, procedures, documentation and processes for each phase of the work, for
which the contractor to ensure compliance with the approved quality control plan.
The RPR will inspect both work in progress and completed work on a daily basis. In addition to
assuring quality workmanship in accordance with the contract documents, the RPR will record quantity
of work place, and will provide daily documentation of construction operations and activities
The RPR will complete all necessary close out and construction completion forms and documentation in
coordination with the County for the project.
FEE PROPOSAL ASSUMPTIONS AND EXCLUSIONS
The aforementioned scope of services assumes that:
1. No Electrical Engineering Design Services are included in this proposal.
2. No Post Design Services are included in this proposal (with the exception of responses to contractor
inquiries during the bidding process).
3. Fees related to permit applications, permit variances, etc. will be provided by the County.
4. The County shall provide boring test results for the segment along CR 905 from the intersection with US 1
north for approximately 1.9 miles. No borings shall be performed along this area by EAC under this
contract..
5. No Right of Way takes are required.
6. The County will provide As- Builts where available for utilities within the project limits that are owned and
maintained by the County.
7. Utilities shown on the plans will be limited to those identified through Sunshine State One Call of Florida
and those which have been provided via As- Builts from Monroe County.
8. No underground subsurface utility engineering (SUE) exploration services are included in the basic scope
of services
9. No Roadway Traffic Signalization is included in scope of work /services.
10. Drainage Design for the addition of the bike lanes is included in the scope of services.
11. No de- mucking design, plans and specifications is included in the scope of work.
12. Front end documents for the bid package will be the responsibility of Monroe County, However, EAC will
review the front end specifications for applicability to the project.
13. To facilitate the project schedule, this scope of services assumes that Monroe County will consolidate and
provide review comments within two (2) weeks of submittal (estimated).
14. No improvements within any private parcels or property are included in scope of work /services.
15. No improvements to any utilities in the project vicinity are included in scope of work /services.
16. Project scope is based on improvements specified in the Monroe County provided task order /scope of
services.
17. Project will be designed per FDOT requirements; however, project tasks will be streamlined and adjusted as
necessary to meet Monroe County's schedule requirements where necessary.
18. No Pavement Design Services are included in scope of work. Bike Lanes will be designed to match existing
pavement structure provided there is no damage or failure to the existing pavement. If the need for a
pavement design because necessary, EAC will advise Monroe County Engineering accordingly.
19. No Mitigation Plans are included in the base scope of work/services.
COMPENSATION
Prime Consultant Compensation will be based on a time and materials basis with not to exceed amounts for the tasks.
Negotiated rates per staff classification will be utilized for invoices including staff hours for respective staff. Invoices shall
include all reimbursable expenses within the invoicing period. Sub Consultant Compensation will be billed as a direct
expense on invoices.

Design Development (30% Phase) $ 56,139.00
