Should you have any questions, please do not hesitate to contact this office. cc: County Attorney Finance File/ TASK ORDER FOR ON CALL PROFESSIONAL SERVICES BETWEEN MONROE COUNTY AND TY LIN INTERNATIONAL/ H J ROSS FOR BLIMP ROAD LAUNCH RAMP- CUDJOE KEY In accordance with the Continuing Contract for On Call Professional Engineering Services made and entered on the 20th day of January 2010 between Monroe County hereinafter referred to as the "County" and TY LIN INTERNATIONAL/ H. J. ROSS hereinafter referred to as "Consultant" where design services are allowed if construction costs do not exceed $2,000,000. All terms and conditions of the referenced Contract For On Call Professional Engineering Services apply to the Task Order, unless the Task Order modifies an Article of the Agreement of which will be specifically referenced in this Task Order and the modification shall be precisely described. This Task Order is effective on the /L'A Day of M 2012. Article Il Scope of Basic Services, Paragraph 2.1.1 is amended as follows: The scope of services for the Blimp Road Launch Ramp Project on Cudjoe Key (Bay Side) will include completion of design for construction and any required permitting to repair the existing launch ramp at the end of Blimp Road. The repair will require the construction of a concrete ramp approximately 12` width x 30' length. The Design for Construction shall include, but shall not necessarily be limited to, plans and specifications which describe all systems, elements, details, components, materials, equipment, and other information necessary for construction. The Design for Construction shall be accurate, coordinated and in all respects adequate for construction and shall be in conformity, and comply, with all applicable law, codes, permits, and regulations. Products, equipment and materials specified for use shall be readily available unless written authorization to the contrary is given by the County. Details of the scope of services are outlined below and included in the Consultant's proposal that is attached to this task order as Exhibit A. 1.0 PROBABLE COST OF CONSTRUCTION ESTIMATE The Consultant shall submit to the County in writing its final estimate of the contractor's anticipated bid price for constructing each of the (2) construction methods. The County shall use this information to evaluate the most cost - effective solution for the repair. The two methods are: 1) Pre - fabricated concrete sections to be assembled on site. 2) Cast - in -place concrete slab. The following Construction Estimate requirements shall apply to each of the two methods. Blimp Road Launch Ramp TY LIN International/ H. J. Ross March 2b, 2012 Pagel of4 1.1 The Construction Cost shall be the total estimated bid cost to the County of all elements of the Project designed or specified by the Engineer. 1.2 The Construction Cost shall include the cost at current market rates of labor and materials and Equipment designed, specified, selected or specially provided for by the Engineer, plus a reasonable allowance for Contractor's overhead and profit. 1.3 Construction cost does not include the compensation of the Consultant and the sub - consultants, the costs of land, rights -of -way, financing or other costs which are the responsibility of the County. 1.4 The Engineer agrees that, should the bid for construction of the project exceed its estimate by ten percent (10%) or more, it will redesign, redraw or rebid, at no additional cost or expense to the County, until the bids are within the stated limits. 2.0 BATHYMETRIC SURVEY The Consultant shall provide a survey of the launch ramp area. 2.1 The bathymetric survey shall be used as the basis of the design. 2.2 The Consultant shall submit the survey with the permit application /s to necessary agencies to meet regulatory permit requirements. 3.0 GEOTECHNICAL INVESTIGATION The Consultant shall conduct a soil assessment of the launch ramp area to determine the stability and bearing capacity. 3.1 The installation of (2) shallow soil borings will be required. 4.0 LAUNCH RAMP REPAIR DESIGN The Consultant shall prepare the design of the selected method of construction by the County to repair the launch ramp. The Consultant shall provide draft drawings and applicable Technical Specifications for the County's review. 4.1 The Consultant shall prepare Construction Documents consisting of Drawings and Specifications setting forth in detail the requirements for the construction of the project. 4.2 Upon completion of the Construction Documents Phase, the Consultant shall provide Final Construction Documents for the County's approval. Upon approval by the County, the Consultant shall provide the County up to 5 sets of Construction Documents that have been signed and sealed by the Engineer. The Consultant shall also provide an electronic version of the construction documents. 4.3 The Consultant shall assist the County in the preparation of the necessary bidding information for the production of bidding forms, the Conditions of the Contracts, and the forms of Agreements between the County and the Contractors by providing supporting information as to the projects scope, bid items, estimated quantities and Blimp Road Launch Ramp TY L1N ]nternationall H. The County shall prepare all Bidding Forms, Conditions of the Contract, and Forms of Agreement. 5.0 AGENCY COORDINATION AND PERMITTING The Consultant shall coordinate with all appropriate Federal, State, and Local government agencies to satisfy all requirements to obtain necessary permits. 6.0 CONSTRUCTION PHASE SERVICES The Consultant shall provide construction phase services that include site visits during construction, review of all shop drawings, responses to all Requests for Information (RFI), and project close out. Article VII, Paragraph 7.1 is amended to include the following: The Consultant shall be paid monthly for a not to exceed total amount of $24,831.00 based on the rates of the Continuing Engineering Services Contract and estimated staff hours as follows: Personnel Rate Estimated Hours Project Manager $196.00 10 Chief Engineer $196.00 4 Senior Engineer $145.00 24 Project Engineer $110.00 64 Senior Technician $81.00 16 Technician $58.00 48 Article IX Miscellaneous, Paragraph 9.29 Federal Highway Administration Requirements do not apply to this project. Monroe County is requesting design and permitting services to perform the needed repairs, which involves extending the existing ramp's apron. The services to be provided include a preliminary estimate of probable construction cost, design of needed repairs, processing of the project through the regulatory permitting process, and construction phase engineering services in accordance to acceptable industry standards. The launch ramp facility will be prepared in accordance with the applicable Monroe County Code Land Development Regulations and both the US Army Corps of Engineers' (USACE) and Florida Department of Environmental Protection's (FDEP) criteria for a general permit or exemption. 2.0 SCOPE OF BASIC SERVICES TY Lin I HJ Ross will prepare the design of the launch ramp repairs limited to the following tasks: 2.1 Probable Cost of Construction Estimate Cost estimates for two (2) alternatives will be prepared to aid in the County's decision on the preferred construction method that provides the most cost - effective solution. The two alternatives consists of: 1) Pre- fabricated slabs per the sketches provided by the City; and 2) Cast -in -place slab. 2.2 Bathyrnetric Surveys The surveys of the proposed ramp areas will be prepared and used as the basis for the design of the project in meeting federal, state, and local policies and regulations. The surveys will be sufficient for submittal with required permit applications. 2.3 Geotechnical Investigation A soil report with an assessment of the bearing capacity of the area of the proposed ramp and its stability will be performed. This will involve the installation of two (2) shallow soil borings. 2.4 Launch Ramp Repair Design TY Lin I HJ Ross will prepare the design for the needed repairs to the launch ramp as per the alternative selected by the County. The repairs will include extending the launch ramp and matching the existing so that it is flush. Draft plans will be presented to the County and finalized based on the County's review comments. It is anticipated that a general permit or exemption will be applied to this project This task will be limited to the total hours included in the Fee I✓stir P ermitting and Coordinati Exhibit B. 2.6 Construction Phase Services TY Lin I HJ Ross will perform construction phase services to consists of two (2) site visits during construction, review of shop drawings, responses to Request for Information (RFI) (limited to a total of two responses), and project close out. 3.0 ITEMS NOT INCLUDED 3.1 Permit Fees 3.2 Mitigation Plans 3.3 Bid & Award Services 4.0 INFORMATION TO BE PROVIDED TO TV LIN I HJ ROSS 4.1 Record Drawings and As -Built plans, including any CAD files, that may be available 4.2 Copies of any submerged land leases obtained by the County adjacent to the project 4.3 Limits of County -owned property adjacent to the project 4.4 Access to the site 4.5 Any pertinent information available 5.0 ADDITIONAL SERVICES The above scope of services represents "basic services" to be performed as part of our lump sum price. Additional services beyond the scope of these basic services will only be performed on an as needed basis and with approved authorization.
