Monroe County
Key West International Airport
Pavement Milling and Overlay of Feraldo Circle
SCOPE OF WORK
Jacobs will provide design and bidding services for the pavement rehabilitation project of Feraldo
Circle at the Key West International Airport (EYW). The rehabilitation shall consist of pavement
milling and a hot mix asphalt overlay. These data gathering, design and associated services will
provide the following:
Article A — Data Collection
Coordinate with survey and geotechnical subconsultants.
Perform a professional topographic survey of the existing Airport access road, Feraldo
Circle, see attached sketch:
o A 17.5 foot by 50 foot grid for the roadway pavement and a 50 foot by 50 foot
grid for turf area shall be surveyed.
o Survey shots taken on pavement will have an accuracy of 0.01' and those on turf
will be 0.1'.
o Grade breaks within the grid will be surveyed.
o All major site features (i.e. structures, manholes, drainage structures,
swales /ditches ponds concrete duct markers, lights, foundations, etc.) will be
located. Ground elevation and top of feature elevation will be surveyed.
o Location and elevation of geotechnical bore locations.
o Survey information will be provided in Autodesk Land Desktop 2004 format.
Survey information will be separated into multiple layers (i.e. drainage,
pavement marking, building, major site features, contours elevations, point
markers, descriptions, etc).
Research record drawings for underground utilities, pavement limits and typical sections.
Perform two (2) site walk through survey to locate and note any special site conditions
that would affect construction techniques or materials and to field verify that all existing
features in the field were picked up during the actual survey.
Perform a geotechnical investigation to provide data for the roadway pavement and
design. The geotechnical investigation includes:
o Two asphalt cores with SPT borings to a depth of 3'.
o Four asphalt cores, cut/patch.
Services for Article A will be provided under a lump sum basis.

Based on the information collected under Article A and constraints identified in Article B, the
60 %, and final contract documents design will be developed for all elements identified in this
scope of work.
• Provide construction plans, including traffic control plans, erosion control plans for
pavement milling and overlay project for Feraldo Circle at Key West International
Airport in Key West Florida.
• Provide specifications and contract documents in accordance with current Monroe
County standards.
• Coordinate and incorporate Monroe County provided contract document /front -end
specifications into the final contract bid documents. (i.e. Notice to Bidders, Invitation to
Bidders, Contract, etc.)
• Prepare for and attend two (2) meetings at the Airport to discuss project progress and
review the design submittals.
• Prepare for and attend one (1) meeting at the Airport to discuss construction phasing with
Airport Staff and Tenants.
• Provide a detailed construction cost estimate for the project at the 60% and at final
contract document design levels
• Prepare FAA construction management plan
• Develop the following anticipated plans.

Conduct an in -house QA /QC review prior to each submission. The in -house QA /QC
check will involve an experienced independent individual, depending on each discipline,
to conduct a comprehensive check on all documents to be submitted (i.e. plans,
specifications, estimates, etc.). Time will be required for engineers and CADD
technicians to correct items listed under the in -house QA /QC review.
Address final comments from 60% design for final contract documents.
Services for Article B will be provided under a lump sum basis.

Article C — Advertising and Bidding.
1. Prepare twenty five (25) sets of contract documents and project manual for potential
bidders.
2. Prepare an "Invitation to Bid ", all forms for advertisement, bid proposals, contract bonds,
labor and DBE requirements and other contract documents needed to solicit public bids
for the construction of the project. The Owner shall be responsible for the cost and
placement of all public advertisements required including all newspapers in which the
advertisement is placed.
3. Prepare for and attend the Pre -Bid Conference, one (1) day, and Bid Opening, one (1)
day.
4. Prepare and respond to Bidders questions.
5. Issue necessary Addenda and Directives to Bidders. One (1) addendum is assumed.
6. Review the bids received, prepare the final bid tabulation, and make
recommendations /rejections of award of the Contract to the Owner.
7. Maintain a plan holder's list during the bidding phase which will be updated accordingly.
8. Prepare conformed contract drawings and specifications.
Services for Article C will be provided under a lump sum basis.
ASSUMPTIONS
Services not included in our scope of work include, but are not limited to, the following:
• Any Permitting.
• Construction phase services.
• Pavement design is not included.
• Scope of Work is based on the assumption that the Geotechnical Investigation
will confirm the rehabilitation technique to consist of a mill and overlay.

Feraldo Circle
Key West International Airport
American Infrastructure Development, Inc. (AID) will provide Professional Services to
JACOBS for the design and bidding phases of the rehabilitation of the asphalt pavement
along Feraldo Circle at Key West International Airport (EYW).
General Information
Feraldo Circle consists of the entrance loop road to the terminal at EYW. The asphalt
pavement on this road is over 20 years old and is showing accelerated levels of
deterioration. Based on current data available, the pavement can be rehabilitated by
milling the existing asphalt and replacing with new bituminous concrete pavement.
Design Criteria
This project will be designed according to Monroe County design standards and FDOT,
as applicable. Construction drawings will be prepared in AutoCAD 2013 format.

The project limits, as shown below, include the terminal loop entrance road along the
arrival area. The pavement section varies in width from one lane (at departure /arrival
connection) to up to four lanes in front of the terminal.

SCOPE OF WORK
General Scope
The Project Team will provide the following general services for this project:
• Coordination with Survey and Geotechnical Consultants.
• Coordination with JACOBS and EYW, as required.
• Preparation of minutes of meetings.
• Program verification and site investigation.
• Design and preparation of the construction and front -end contract documents.
• Preparation of the Engineer's Report and construction cost estimate.
• Bidding and award assistance.
Specific Scope
The Project Team will prepare construction documents for the rehabilitation of the
bituminous concrete pavement along Feraldo Circle. The project will be advertised
using a Base Bid. Alternate Bids are not anticipated.
Specific tasks related to the above items are identified in each phase of the project as
described below.
Data Collection
AID will perform the following tasks under this phase:
• Compile and review existing available information including as -built drawings,
geotechnical reports, pavement evaluation report, and maintenance logs.
• Perform a detailed site inspection and evaluate the existing condition of the
pavements.
• Coordinate with the survey and geotechnical Team members.
• Meet and coordinate with JACOBS and EYW on project issues such as design
alternatives, project phasing, construction staging, budget, and schedule.
• Identify any additional information that may be required from field investigations
or other agencies.
Contract Documents (60% and Final)
The Project Team will proceed with the design and preparation of contract documents
for this project including the evaluation of up to two pavement rehabilitation alternatives.
Construction documents will be prepared in this phase, including finalizing the
construction phasing plan, which may include day -time and night -time construction
activities. At this stage, the construction cost estimate and construction schedule will be
updated and finalized and the Engineer's Report will be completed. The Project

Manual, which will contain front -end documents (Instructions to Bidders, Bid Forms, and

comments regarding the design of this project.
Upon receipt of final comments from JACOBS and EYW, AID will proceed with the
preparation of the bidding documents. This effort includes incorporating comments by
updating the construction drawings, project manual (front -end documents, general
provisions, and technical specifications), Engineer's Report, construction cost estimate,
and construction schedule.
AID will submit electronic copies of the 100% contract documents to JACOBS for
distribution during the bidding phase.
Phase 4 — Bidding and Award Services
This phase will include the effort necessary to advertise for and receive bids from
contractors and to review the bids and make a recommendation of award to the Owner
and the FAA. Specifically, the following tasks will be performed by AID during this
phase:

Topographic Survey of Feraldo Circle
Dear Mr. Nielsen:
In accord with your request, we offer our proposal to perform a topographic survey to support
the design of the milling and overlay of asphalt paving on the access loop road Feraldo Circle
servicing the terminal at Key West International Airport comprised of approximately 1,300
linear feet, commencing and terminating at South Roosevelt Boulevard.
Limits
The limits of the survey shall be along transects taken at 50 foot intervals from the existing edge
of asphalt to the existing edge of asphalt on the opposite side, including centerline of paving,
edge of curb, flowline, top of curb, edge of walk and top of walk. Changes of direction, highs
and lows, including significant changes in grades shall also be collected.
Scope
The survey minimum requirements for the project are as follows:
1. Establish benchmarks on site to perform the topographic cross - sections. The benchmarks
shall be established by a closed -loop three wire differential leveling from a National
Geodetic Survey benchmark.

2. Elevations shall be based on the National Geodetic Vertical Datum of 1929 (NGVD 29)
as referenced to the above benchmark by the National Geodetic Survey (NGS). The
elevations shall also be horizontally referenced to the 1990 adjustment of the North
American Datum of 1983 (NAD 83 -90).
Deliverables
1. Two signed and sealed hard copies of the survey shall be furnished to the client, together
with an AutoCAD (2008 or lower) digital drawing file created in Autodesk Land
Development Desktop, including a PDF file of the hard copy.
2. The drawing scale shall be at 1 " =30'.
Timeframe
We can mobilize to the site within 48 hours from a notice to proceed and have the project
completed within two weeks after mobilization.
Cost
The cost to perform the above services shall be a lump sum amount of $5,810 which includes
mobilization and demobilization.
General Conditions
All work shall be completed under the direction of a Professional Land Surveyor registered in
the State of Florida. All work shall be conducted using equipment, personnel, and procedures
that will insure compliance with the accuracy standards as defined by State of Florida Minimum
Technical Standards in Chapter 5J- 17.05, FAC. All documents submitted shall bear the
surveyor's seal, signature, and a certificate that all work was done under the surveyor's
supervision and that all information contained in the document is true and is accurately shown.
The surveyor is responsible for quality assurance for the survey work performed on this project.
All survey information shall be recorded in hard bound field books or in electronic data collector
files. A map shall be provided by the surveyor in AutoCAD, Version 2008 (or earlier version).

Dear Eric:
As requested, Tierra South Florida, Inc. (TSF) is pleased to submit this proposal for the above -
referenced project. The proposal is based on e -mail from Jacobs dated July 11, 2012 and follow
up discussions.
PROPOSED SCOPE OF WORK
All work performed by TSF will be in general accordance with Advisory Circular AC 150/5320 -
6D. Geotechnical Investigation Services will include two (2) asphalt cores with Standard
Penetration Test (SPT) borings to a depth of 3 feet, and four (4) asphalt cores. The cores and
borings will be performed at the approximate locations provided or approved by Jacobs;
The following laboratory testing will be performed on selected samples:
Moisture Content test (ASTM D2216) — 1 test
Grain size analysis (ASTM D422) — 1 test
Organic Content tests (AASHTO T -194) — 1 test
Borings and test locations will be located in the field by Jacobs. It is our understanding that
elevations at boring locations will also be provided by Jacobs.
Prior to drilling at the project site, TSF will notify the local utility companies and request that
underground utilities be marked. Our experience, however, is that the utility companies will not

mark privately owned utilities. We have included Ground Penetrating Radar (GPR) in our
estimate to clear utilities at the boring locations. It is important to note that while the use of GPR
will minimize the potential of striking utilities, it is not a 100% accurate. Therefore, private
utilities will be the owner's /Airport's responsibility. We believe that significant coordination will
likely be required to perform the field operations. As we understand, the all work will be
conducted in the day time. It is our understanding that Jacobs or the Department of Aviation will
provide an escort to the test locations.
Upon completion of the field exploration, laboratory testing will be performed on selected
samples.
A report will be issued that contains the exploration (pavement core) data.
ESTIMATED FEES
It is proposed that the fee for the performance of the services outlined above is determined on a
unit price basis in accordance with the attached Fee Schedule and that the work be performed
pursuant to TSF's General Conditions enclosed herewith and incorporated into this proposal. On
the basis of the proposed quantities, it is estimated that the total fee will be approximately
$5,345.00.
Our estimate covers the work needed to present our findings and recommendations in a formal
report. Not included are reviews of foundation drawings, preparation of construction
specifications, special conferences and any other work requested after submittal of our report.
Boring, sampling, and testing requirements are a function of the subsurface conditions
encountered. Therefore, the estimated fee previously indicated is approximate, and
compensation for the exploration will be based on the actual work and tests performed. We will
endeavor to keep the exploration cost at a minimum consistent with good engineering practice.
SCHEDULE AND AUTHORIZATION
TSF will proceed with the work after receipt of a signed copy of this proposal. With our present
schedule, we can commence work within several days of project approval (weather permitting)
and fieldwork is expected take about 1 day to complete. The written report can be submitted

within two weeks after completion of the field exploration, depending on the extent of the
laboratory- testing program. Verbal results can be made available to appropriate parties prior to
submittal of the written report.

Tierra South Florida's General Conditions
1. SCOPE OF WORK: Work means the specific geotechnical, analytical, testing or other service to be performed by Tierra South Florida, Inc. (TSF) as set forth in TSF's proposal, Client's acceptance of the scope of
work and these General Conditions. Additional work ordered by Client shall also be subject to these General Conditions. "Client" refers to the person or business entity ordering the work to be done by TSF. Client
shall communicate these General Conditions to each and every third party to whom Client transmits any part of TSF's work. TSF shall have no duty or obligation to any third party greater than that set forth in TSF's
proposal, Client's acceptance of TSF's proposal and these General Conditions. The ordering of work from TSF, or the reliance on any of TSF's work, shall represent acceptance of the terms of TSF's proposal and
these General Conditions, regardless of the terms of any subsequently issued document.
2. RIGHT -OF -ENTRY -The client will provide right -of -entry for TSF and all necessary equipment in order to complete the work. While TSF will take all reasonable precautions to minimize any damage to the
property, it is understood by Client that in the normal course of work some damage may occur; the correction of which is not part of this agreement.
3. DAMAGE TO EXISTING MAN -MADE OBJECTS -The Client, will provide the location of all underground utilities or obstructions to TSF who, in the prosecution of their work, will take all reasonable
precautions to avoid damage or injury to any such subterranean structure or utility. The Owner agrees to hold TSF harmless for any damages to subterranean structures which are not called to TSF attention and
correctly shown on the plans furnished and will reimburse TSF for any expenses in connection with any claims or suits including reasonable attorney fees at the trial and appellate levels.
4. IN -PLACE MATERIALS TESTING -TSF will not be responsible for repair or damage to portions of structures designated for in -place materials testing. Repairs can be made for aesthetic reasons if requested in
advance of the work to be performed. The cost for labor and materials would be charged.
5. SAMPLE RETENTION -TSF will retain all soil and rock samples obtained for geotechnical explorations for 30 days. Samples subjected to Construction Materials and Laboratory testing are disposed of subsequent
to testing. Further storage or transfer of samples can be made at Client's expense upon written authorization.
6. DEFINITION OF RESPONSIBILITY (OBSERVATION SERVICES) - The presence of our field representative will be for the purpose of providing observation and field testing. Our work does not include
supervision or direction of the actual work of the contractor, his employees or agents. The contractor for this project should be so advised.
The Contractor should also be informed that neither the presence of our field representative or the observation and testing by our firm shall excuse him in any way for defects discovered in his work. It is
understood that TSF will not be responsible for the Contractor's job or site safety on his project. That will be the sole responsibility of the contractor.
7. STANDARD OF CARE - Service performed by TSF under this Agreement will be conducted in a manner consistent with that level of care and skill ordinarily exercised by members of the profession currently
practicing under similar conditions. No other warranty, expressed or implied, is made.
7.1. Client recognizes that subsurface conditions may vary from those encountered at the location where borings, surveys or explorations are made by TSF and that the data, interpretations and recommendations
of TSF are based solely on the information available to it. TSF shall not be responsible for the interpretation by others of information developed.
8. ORAL AGREEMENTS -No oral agreement, guarantee, promise, representation or warranty shall be binding.
9. OWNERSHIP OF DOCUMENTS -AIL reports, boring logs, field data and notes, laboratory test data, calculations, estimates and other documents prepared by TSF, as instruments of service, shall remain the
property of TSF until final payment is received and a letter of copyright transfer been executed.
10. BASIS OF PAYMENT - Payment is due within 30 days of date of invoice. Payments not made when due shall bear interest at eighteen (18) percent annum or at the maximum rate allowed by law from the date of
the invoice until same is paid.
10.1. If the Client fails to make any payment due to TSF for service and/or expenses within 60 days of date of invoice, TSF may, after giving seven days' written notice to Client, suspend services until all
outstanding amounts have been paid to TSF in full. Further, TSF may, in addition to withholding services, or singularly, withhold reports, plans and other documents not paid in full by the Client. In the
event that final payment for completed work is not made, TSF shall request that all copyrighted documents which were submitted to client be returned and all information used in project plans be removed
from project documents.
10.2. In the event it is necessary to take legal action to effect collection, whether or not litigation is commenced, the Client agrees to reimburse TSF for expenses in connection with any claims or suits, including
reasonable attorney's fees, including but not limited to the trial and appellate levels.
103. This contract shall be governed by the laws of the State of Florida.
L CONSTRUCTION REVIEW - TSF cannot accept responsibility for any design work unless the work includes services for construction review to determine whether or not the work performed is in substantial
compliance with TSF's conclusions and recommendations.
12. INDEMNIFICATION -TSF agrees to hold harmless and indemnify Client from and against liability arising out of TSF's negligent performance of the work. Client agrees to indemnify and hold TSF harmless from
all liability including all costs, attorney's fees and expenses of defense for any claims by any other person or corporation which may arise out of the performance or breach of this contract for which TSF was not
solely negligent.
13. LIMITATION OF LIABILITY -The Client/Owner agrees to limit TSF liability for negligent professional acts, errors or omissions, such that the total aggregate liability of TSF shall not exceed $50,000 or the total
fee for the services rendered on this project; whichever is greater. The Owner further agrees to require the contractor and his subcontractors a similar limitation of liability suffered by the contractor or the
subcontractors arising from TSF negligent professional acts, errors or omissions.
13.1. If Client prefers to have higher limits on professional liability, TSF agrees to increase the limits up to a maximum of $1,000,000 upon Client's written request at the time of accepting our proposal provided
that Client agrees to pay an additional consideration of 5 percent of our total fee. The additional charge for the higher liability limits is because of the greater risk assumed and is not strictly a charge for
additional professional liability insurance.
14. INSURANCE -TSF represents and warrants that it and its agents, staff and consultants employed by it are protected by Worker's Compensation insurance and Employer's Liability Insurance in conformance with
applicable state laws. TSF has such coverage under public liability and property damage insurance policies that TSF deems to be adequate. A Certificate of Insurance can be supplied evidencing such coverage upon
request.
14. L Within the limits and conditions of such insurance, TSF agrees to indemnify and save client harmless from and against any loss, damage or liability arising from any negligent acts by TSF, its agents, staff
and consultants employed by it. TSF shall not be responsible for any loss, damage or liability beyond the amounts, limits and considerations of such insurance. TSF shall not be responsible for any loss,
damage or liability arising from any acts by clients, its agents, staff and other consultants employed by it.
14.2. Cost of the above coverage is included in our quoted fees. If additional coverage or increased limits of liability are required, TSF will endeavor to obtain the requested insurance and charge separately for
costs associated with additional coverage or increased limits.
15. TERMINATION -This agreement may be terminated by either party upon seven days written notice in the event of substantial failure by the other party to perform in accordance with the terms thereof Such
termination shall not be effective if the substantial failure has been remedied before expiration of the period specified in the written notice. In the event of termination, TSF shall be paid for services performed to the
termination notice date plus reasonable termination expenses.
15.1. In the event of termination or suspension for more than three months, prior to completion of all reports contemplated by this Agreement, TSF may complete a report on the services performed to the date of
notice of termination or suspension. The expenses of termination or suspension shall include all direct costs for TSF in completing such analyses, records and reports.
16. CLIENT'S OBLIGATION TO NOTIFY TSF - Client represents and warrants that it has advised TSF of any known or suspected hazardous materials or conditions, utility lines and pollutants at any site at which
TSF is to do work hereunder, and unless TSF has assumed in writing the responsibility of locating subsurface objects, structures, lines or conduits, Client agrees to defend, indemnify and save TSF harmless from all
claims, suits, losses, costs and expenses, including reasonable attorney's fees as a result of personal injury, death or property damage occurring with respect to TSF's performance of its work and resulting to or
caused by contact with subsurface or latent objects, structures, lines or conduits where the actual or potential presence and location thereof were not revealed to TSF by Client.
17. HAZARDOUS MATERIALS -This agreement shall not be interpreted as requiring TSF to assume the status of an owner, operator, generator, storer, transporter, treater or disposal facility as those terms appear
within RCRA or within any Federal or State statute or regulation governing the generation, transportation, treatment, storage and disposal of pollutants.
