At the November 20, 2012, Board of County Commissioners meeting, the Board granted
approval and authorized execution of Item F17 Contract with C &W Pipeline, Inc. for the
Doctor's Arm Culvert Repairs Project located on Big Pine Key.
At the December 12, 2012, meeting the Board granted approval and authorized execution
of the following:
Item C14 Amendment 1 to the Task Order with AMEC Environment & Infrastructure, Inc.
(Amec) for Construction Engineering and Inspection Services for the Tom's Harbor Channel
Bridge Project to extend the completion date.
✓ Item C15 Amendment 1 to the Task Order with EAC Consulting, Inc. for design and
permitting services for the CR 905 bike lane project to add additional geotechnical engineering
services to the scope of services. The additional services result in a fee increase of $13,876.80.
Enclosed is are two duplicate originals of each of the above - mentioned for your handling.

AMENDMENT ONE TO THE TASK ORDER FOR ON CALL PROFESSIONAL
SERVICES BETWEEN MONROE COUNTY AND EAC CONSULTING, INC.
FOR
CR 905 BIKE LANES PROJECT DESIGN
THIS TASK ORDER AMENDMENT IS ENTERED INTO BY Monroe County hereinafter
referred to as the "County" and EAC Consulting, Inc., its successors and assigns,
hereinafter referred to as "Consultant". The amendment is effective as of this 12"' day
of December, 2012.
Article II Scope of Basic Services, Paragraph 2.1.1 is amended as follows:
The CONSULTANT will perform additional geotechnical engineering and material testing
services for the COUNTY as described in Attachment 1 to this amendment.
Article VII, Paragraph 7.1 is amended to include the following:
The Consultant shall be paid monthly on a time and material basis as described in
Attachment 1; the additional not to exceed amount of $13,876.80 will apply for the
additional services.
All requirements of the original task order and this amendment shall be completed no
later than April 30.2013.
All other terms and conditions of the Contract For On CaII Professional Engineering
Services dated January 20, 2010 and the Task Order dated April 18, 2012 remain in
effect.

Dear Ms. Clarke;
EAC Consulting Inc. (EAC) is pleased to submit this supplemental fee proposal to Monroe County
Engineering for civil engineering services on the above referenced project.
Further to our comments resolution meeting held on October 18, 2012 it was agreed that Limerock
Bearing Ratio (LBR) tests be conducted within the limits of the project to determine if the existing
material on -site has sufficient strength to avoid the need for limerock stabilization of the subgrade.
In the event that limerock stabilization can be eliminated from this project through testing and
analyses as proposed within this scope of works, the County could potentially realize significant
cost savings in subgrade stabilization costs, maintenance of traffic costs, and existing asphalt
restoration costs.
Our services on this project will be supported by the following sub - consultants:
• Wingerter Laboratories, Inc. (WL) — Geotechnical Engineering and Material Testing
The anticipated scope of work is shown in Attachment 1. EAC will be providing coordination
services with WL and will analyze the test results for incorporation within the on -going design of the
referenced project.

ATTACHMENT 1
SCOPE OF SERVICES
Background
Monroe County has requested that EAC Consulting, Inc. (EAC)., provide a fee proposal to
undertake material testing related to the design of five -foot (5') wide bicycle lanes located within
eight -foot (8') wide roadway shoulders along CR 905 from Ocean Reef, south to the intersection
with US 1 (approximately 10.5 mi.) in Key Largo, Monroe County, Florida.
Accordingly, the following project tasks are required to undertake the requisite professional
engineering services for the project are described as follows:
TASK 1: GEOTECHNICAL ENGINEERING AND MATERIAL TESTING
Task 1.1 — Project Management and Coordination
EAC will provide Project Management and Coordination Services that will comprise of contract
administration, preparation of invoices, coordination of sub consultant's work, and monitoring
the progress of the works associated with this proposal.
Task 1.2 — Limerock Bearing Ratio (LBR) Test
FM 5 -514: Includes laboratory testing by a certified FDOT /ACI Aggregate Technician, sample
preparation, Gradation, Testing, and a typed report with LBR graph and moisture curve.
Task 1.3 — Sieve Analysis (Gradation)
ASTM C -136, FDOT, FAA F -211 — Includes: Includes laboratory testing by a certified FDOT /ACI
Aggregate Technician, sample preparation, sieve analysis testing and a typed report.
Task 1.4 — Qualified Sampler & Equipment
Provide the services of a CTQP /FDOT Qualified Sampler to obtain aggregate samples for
laboratory testing. Logs of sampling hours and activities will be documented in a report. Provide
the services of a qualified heavy equipment operator to dig out samples out approximately forty -
four locations along a 10.5 mile strip.
Task 1.5 — Recommendation Report
Provide a report summarizing all test results. Provide specific recommendations on whether the
5 -foot bike path and the 8 -foot shoulder as shown on the typical section can be constructed with
or without the need for 12 -inch stabilization and that the material to depth of 20- inches below
existing grade meets a minimum of LBR -40 for entire limits of the project. In addition, the
recommendations will include any specific subgrade and site preparations that may be required
by the contractor, if any. Report will be signed and sealed by a Florida Registered Professional
Engineer.
Task 1.6 — Incorporation of Test Result in Construction Documents
Based on the results of the above, EAC will incorporate the recommendations within the
Construction Documents.
FEE PROPOSAL ASSUMPTIONS AND EXCLUSIONS
The aforementioned scope of services assumes that:
1. No Electrical Engineering Design Services are included in this proposal.
2. No Post Design Services are included in this proposal
3. Fees related to permit applications, permit variances, etc. will be provided by the
County.
4. No Right of Way takes are required.
5. The County will provide As- Builts where available for utilities within the project limits
that are owned and maintained by the County.
6. Utilities shown on the plans will be limited to those identified through Sunshine State
One Call of Florida and those which have been provided via As- Builts from Monroe
County.
7. No underground subsurface utility engineering (SUE) exploration services are included
in the basic scope of services
8. No Roadway Traffic Signalization is included in scope of work/services.
9. No de- mucking design, plans and specifications is included in the scope of work.
10. Front end documents for the bid package will be the responsibility of Monroe County,
however, EAC will review the front end specifications for applicability to the project.
11. No Pavement Design Services are included in scope of work. Bike Lanes will be designed
to match existing pavement structure provided there is no damage or failure to the existing
pavement. If the need for a pavement design because necessary, EAC will advise Monroe
County Engineering accordingly.
12. No Mitigation Plans are included in the base scope of work/services.
COMPENSATION
Prime Consultant Compensation will be based on a time and materials basis with not to exceed
amounts for the tasks. Negotiated rates per staff classification will be utilized for invoices including
staff hours for respective staff. Invoices shall include all reimbursable expenses within the
invoicing period. Sub Consultant Compensation will be billed as a direct expense on invoices.
