Should you have any questions, please feel free to contact my office. cc: County Attorney Finance File Amendment 001 CONTRACT AA -1329 El ORIGINAL page 1 THIS AMENDMENT is entered into between the Alliance for Aging, Inc. hereinafter referred to as the "Alliance ", and Monroe County Social Services. The purpose of this amendment is to increase the allocation under title IIIB by $31,000 as a result of the 2012 ITN "Empowering Older Adults through Internet Access ". This amendment also modifies the provider's contract to add the following section to Attachment I of the original contract 2.7.8 Empowering Older Adults Through Internet Access Contractors funded under the Empowering Older Adults Through Internet Access ITN must submit a plan to the Alliance prior to spending funds. The plan will detail how the funds will be spent and how the effectiveness of these expenditures will be measured. The Alliance will reimburse only for expenditures that are made according to the plan. All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the contract are hereby amended to conform with this amendment. This amendment and all its attachments are hereby made a part of the contract.
