WHEREAS, the PROVIDER is a not-for-profit corporation providing assisted living facilities and services such as mental health support, transportation, and supervision for disadvantaged, mentally ill persons, and WHEREAS, the County and the PROVIDER entered into an Agreement on October 17, 2014 for the provision of the services as set forth above, but inadvertently Attachment C which sets forth the specific services to be provided under the Agreement was omitted; and WHEREAS, this Amendment revises the Agreement to include Attachment C, now, therefore, IN CONSIDERATION of the mutual promises and covenants contained herein, it is agreed as follows: 1. This Amendment revises the original Agreement to include Attachment C (see attached). 2. In all other respects the Agreement dated October 17, 2014 remains in full force and effect. The target population is individuals with co-occurring substance/mental health disorders AND a history of incarceration. The specific interventions include: • Integrated Screening and Assessment(for co-occurring mental health and substance abuse) disorders via he GAINQ3(Global Assessment and Inventory of Needs) • Support groups(Mental Health and Substance Abuse/Co-Occurring) weekly • Drug testing—performed randomly, if use is suspected.
