AMENDMENT 1 to AGREEMENT
This Amendment 1 to the Agreement dated Octulei 17, 2014, is made and entered into
this 10th day of December, 2014, between the BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, hereinafter referred to as "Board" or "County," and Womankind, Inc.,
hereinafter referred to as "PROVIDER."
WHEREAS, the PROVIDER is a not-for-profit corporation established for the provision of
women's health care; and
WHEREAS, the County and the PROVIDER entered into an Agreement on October 17, 2014
for the provision of the services as set forth above; and
WHEREAS, the PROVIDER has requested to revise Attachment C to amend the services to
be funded under the original agreement; and
WHEREAS, this Amendment revises the Agreement to revise Attachment C, now,
therefore,
IN CONSIDERATION of the mutual promises and covenants contained herein, it_is agreed
as follows:
1. This Amendment revises the original Agreement to revise Attachment C (see
attached).
2. In all other respects the Agreement dated October 17, 2014 remains in full force and
effect.

Womankind is a medical and wellness center providing high-quality family planning, primary
health care, and mental health counseling to people of all income levels.
2. List the services your agency provides.
• Annual gynecological exams and Pap tests
• Clinical breast exams
• Mammogram referrals and vouchers
• Breast health diagnostics and secondary care assistance
• Teen services
• Testing and treatment for sexually transmitted infections
• Screening for diabetes and heart disease
• Colposcopy .

• Menopausal counseling
• Bioidentical hormones
• Minor illness treatments
• Hormone replacement therapy

• Emergency contraception .
• Pre-conception and interconceptional care

3. What specific services will be funded by this request?
Womankind will use HSAB support for 5o%of our rent and varying percentages of the following
staff salaries: executive director, assistant director, and front desk staff.
