AMENDMENT 1 to AGREEMENT
This Amendment 1 to the Agreement is dated October 17, 2014 is made and entered into
this 21st day of January, 2015, between the BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, hereinafter referred to as "Board" or "County," and Samuel House, Inc.,
hereinafter referred to as"PROVIDER."
WHEREAS, the PROVIDER is a not-for-profit corporation established for the provision of
women and children's housing and support, and
WHEREAS, the County and the PROVIDER entered into an Agreement on October 17, 2014
for the provision of the services as set forth above, but inadvertently Attachment C which sets
forth the specific service to be provided under the Agreement was omitted; and
WHEREAS, this Amendment revises the Agreement to include Attachment C, now,
therefore,
IN CONSIDERATION of the mutual promises and covenants contained herein, it is agreed
as follows:
1. This Amendment revises the original Agreement to include Attachment C (see
attached).
2. In all other respects the Agreement dated October 17, 2014 remains in full force and
effect.
IN WITNESS WHEREOF, the parties hereto have caused these presents to be executed as
of theday and year first written above.

ATTACHMENT C
Our Mission is "to provide housing in a nurturing environment for homeless women, women with children, men
with children and intact families and to provide them with care coordination beneficial to their physical, mental,
emotional and spiritual well-being."
Our Vision is "to provide women, men and intact families with care coordination to access all appropriate
services/resources to help them become self-sufficient."
Specifically, funds received from the 2015 HSAB Grant will be used for The Mary Spottswood Women's Center
Emergency Shelter in order to provide the following services:
• General Agency operations such as utilities (i.e. electric, gas, water, sewer, phone), drug testing
- supplies, basic essential items necessary to provide quality of life for clients (i.e. food, personal hygiene
products, cleaning supplies), maintenance and security
• Case Management Services
