The work will include preparation of 2 certified copies of the parcel sketch and legal description for the permanent easement documents as described in Attachment A, coordination with FDOT and subconsultant, including revisions required by FDOT. All work will be conducted in accordance with current FDOT, State and Federal requirements. Article VII, Paragraph 7.1 is amended to include the following: The Consultant shall be paid monthly; the following lump sum amounts will apply for each task (except where noted): Task Description Lump Sum Amount 1 Prepare 2- certified copies of the parcel sketch and legal $2,829.40 description for permanent easement documents as described in Attachment A 2 Project Management/Coordination $1,176.26 Total Sixth Amendment $4,005.66 The legal description exhibit required by FDOT will be completed and submitted to the County no later than February 27, 2015, for a total not to exceed fee of four thousand five dollars and sixty -six cents ($4,005.66). Force and Effect. In all other respects the Contract dated October, 2011 approved at the Monroe County BOCC meeting on October 19, 2011, amended on June 20, 2012, amended on May 15, 2013, amended on November 20, 2013, amended on May 21, 2014 and amended on October 17, 2014 remains in full force and effect. This scope items are based on Mapping criteria and are derived from the FDOT Project Activity 27: Survey Man -Hour spread sheet, attached hereto. In addition to the specific scope items, all work shall be accomplished in accordance with the criteria established by the Departments Highway Field Specifications, Survey Handbook (Survey Procedure Topic No. 550-030 - 101a) (Chapter 20, sec 23 (3)(a), F.S.),CADD Production Criteria Handbook and must comply with the Minimum Technical Standards for Land Surveyors Rule 5J -17 F.A.C., Florida Statue 472.027, the latest's addition of the District VI Survey Standards and Guidelines and any special instructions. The proposed Survey Scope of work includes the following Mapping Survey Service. 29.22 Parcel Sketches A parcel sketch for permanent easements will be prepared. 29.29 Legal Descrirations A legal description for permanent easements will be prepared.
