Ross for the Construction Administration Phase of the Drainage Projects at Grouper Lane in Key Largo and 9th Avenue on Stock Island. This project is funded through the Gas Tax Fund. LIN INTERNATIONAL/HJ. ROSS, hereinafter referred to as "Consultant" where design services are allowed if construction costs do not exceed$2,000,000.00. All terms and conditions of the referenced Contract for On Call Professional Engineering Services apply to this Task Order, unless the Task Order modifies an Article of the Agreement of which will be specifically referenced in this Task Order and the modification shall be precisely described. These additions apply only to the project referenced in this Task Order. The parties agree that the prior On Call Contract dated January 20, 2010 has expired and that TY Lin has been paid all the compensation due for completed work under that contract; and that any services left uncompleted under that contract are now null and void. This Task Order is effective on the 21'm of January,2015. 1. In accordance to Article II SCOPE OF BASIC SERVICES on the On Call Professional Engineering Agreement dated January 16w, 2014, the Consultant will perform services as described below: a. Consultant shall perform Construction Administration Phase services for the completion of the Drainage Improvements projects at Grouper Lane, in Key Largo MM95, and 91h Avenue in Stock Island. Services are to consist of the following activities and deliverables: • Pre-Bid meeting attendance • Pre-Construction meeting attendance and minutes • Weekly site visits per site during construction, Minutes and logs will be provided. • Review of shop drawings • Responses to Request for Information(RFD • Punch list • As-Built plan review • Project close out b. The above Construction Administration services represent "basic services" to be performed as part of the Consultant's price. Additional services beyond the scope of these basis services will only be performed on an as needed basis and with approved 1 authorization. If additional services are required, Consultant will prepare a separate scope of services and labor cost. 2. Article III ADDITIONAL SERVICES on the On Call Professional Engineering Agreement dated January 16w, 2014,paragraph 3.1 shall be amended as follows: a. Section 3.1 shall be amended to add the following requirements: The services described in this Article II are not included in the Basic Services. They shall be paid for by the County as provided in this agreement as an addition to the compensation paid for the Basic Services, but only if approved by the County before commencement and as follows: • Wetland Mitigation Design or Fees • Phase I Environmental Assessments • Environmental Monitoring During Construction • Roadway improvements beyond incidental impacts due to drainage improvement construction • Permit fees • Property Boundary Survey (only topographic surveys of the right-of way) • Geotechnical Investigations (County to provide previously performed studies) • Landscaping • Bid& Award Services other than with is described above 3 Article IV COUNTY'S RESPONSIBILITIES in the On Call Professional Engineering Agreement dated January 16, 2014, shall be amended to add the following requirements: • Record Drawings and As-Built plans, including any CAD files, that may be available • Previous Geotechnical Studies and/or Reports for adjacent sites depicting percolation rates and borings • Limits of County-owned property adjacent to the project • Access to the site • Any pertinent information available 4. Article VII PAYMENTS on the On Call Professional Engineering Agreement dated January 16, 2014, paragraph 7.1.1 shall be amended to add the following requirements: a. Section 7.1.1 requires the County shall pay the Consultant in current funds for the Consultant's performance of this Task Order based on rates as follows: • The Group Lane project will be billed monthly on the basis of the percent of the Construction Phase completed. The agreed percentage of the Construction Phase will be determined by the county's Project Manager. The lump sum fee for the Construction Phase is$16,852.00. • The 9th Avenue in Stock Island project project will be billed monthly on the basis of the percent of the Construction Phase completed. The agreed percentage of the Construction Phase will be determined by the county's 2 Project Manager. The lump sum fee for the Construction Phase is $46,328.00. 5. All requirements for both projects shall be completed no later than December 15, 2015, for a total lump sum amount for the Construction Phases for both projects of $63,I80.00. 6. Attachment A — Consultant Rates on the On Call Professional Engineering Agreement dated January 16,2014 shall be amendment as follows: Two additional Job Classifications shall be added as follows: • Chief Engineer $175.00 Burdened Hourly Rate • Senior Technician$81.00 Burdened Hourly Rate All other terms and conditions of the Task Order shall be in accordance with the On Call Professional Engineering Services contract dated January 16,2014.
