COMPLETE ELEVATOR SERVICE MAINTENANCE

of l�'L!y-. , 2015, between MONROE COUNTY, FLORIDA ("COUNTY"), a political
subdi(fision of the State of Florida, whose address is 1100 Simonton Street, Key West, Florida
33040, and Maverick United Elevator ("CONTRACTOR"), a Limited Liability Corporation,
whose address is 10639 NW 122"d St, Medley, FL 33178.
WHEREAS, the parties hereto did on July 17, 2013 enter into an agreement for complete
elevator service maintenance(hereinafter"Original Agreement"); and
WHEREAS,on July 16, 2014 the BOCC approved the first amendment and renewal agreement;
and
WHEREAS, both parties desire to amend Paragraph 3 A, THE CONTRACT SUM, to add an
additional regular labor rate for a mechanic and helper; and
WHEREAS, both parties desire to amend Paragraph 3 B, THE CONTRACT SUM, to add an
additional emergency overtime labor rate for a mechanic and helper;and
WHEREAS,the parties have found the Original Agreement to be mutually beneficial and;
---- ---WHEREAS, the parties have found the First Amendment and Renewal Agreement to be
mutually beneficial and;
WHEREAS, the parties find that it would be mutually beneficial to enter into this Second
Renewal Amendment Agreement;
NOW THEREFORE, IN CONSIDERATION of the mutual promises and covenants set forth
below,the parties agree as follows:
1. Paragraph 3, THE CONTRACT SUM , shall include the CPI-U adjustment and subsections A
and B are amended to include additional regular and emergency overtime rates for a Mechanic and
Helper.
The County shall pay to the Contractor for the performance of said monthly service on a per
month in arrears basis on or before the 30'h day of the following month in each of twelve months.
The Contractor shall invoice the County monthly for elevator service and maintenance
performed under the Specification contained herein.
A. The Contractor amount shall be as stated by the contractors bid (including annual CPI-U
increases to date)as follows:

2. In accordance with Paragraph 5,TERM OF AGREEMENT of the Original Agreement:
a. The County exercises the option to renew the Original Agreement for the second of the
two (2) terms. This term will commence on August 01, 2015 and terminate July 31,
2016.
b. The Contract amounts are being adjusted .8% in accordance with the percentage change
in the U S. Department of Commerce Consumer Price Index (CPI-U) for all Urban
Consumers as reported by the U.S. Bureau of Labor Statistics and shall be based upon the
CPI-U computation at December 31 of the previous year as follows: Elevator Monthly
Amount from $101.50 to $102.32. Inspection Amount from $203.00 to $204.63, Labor
during regular working hours from $76.13 to $76.74, Regular Labor during working
hours for a Mechanic and Helper $126.00/hr., Emergency service calls from $96.43 to
$97.21 and Overtime Labor during Emergency service calls for a Mechanic and
Helper$189.00/hr..
3. Except as set forth in paragraph 1, 2 and 3 of this Second Renewal Amendment Agreement,
in all other respects, the terms and conditions set forth in the Original Agreement as amended
remain in full force and effect.
IN WITNESS WHEREOF, COUNTY and CONTRACTOR hereto have executed this
Second Renewal Amendment Agreement on the day and date first written above in four (4)
counterparts, each of which shall, without proof or accounting for the other counterparts, be
deemed an original contract.
