At the May 18, 2016 Board of County Commissioner's meeting the Board granted approval and
authorized execution of Item:
C2 Approval of Jacobs Project Management Co.' s Task Order # 12/14-61 for Construction Support
Services during construction of New Storage Hangars at the Florida Keys Marathon International
Airport, in the amount of $135,250.00. To be paid 20% Airport Operating Fund 403 and 80% FDOT
grant.
C4 Approval to renew contract between the Florida Keys Marathon International Airport and Jayne's
Cleaning Service, Inc. for One (1) additional two (2) year period, in the amount of $22,800.00 per year
to be paid 100% from Airport Operating Fund 403.

New Storage Hangars — Phase 1 Construction Support Services
EXISTING CONDITIONS
The eleven shade hangars on the northeast side of RW 25 along Taxiway E have exceeded
their useful life span and are also within the Runway Protection Zone (RPZ) of Runway 25.
These shade hangars will be demolished after the construction of the eleven new hangars on
the southeast side of RW 25 along Taxiway A, therefore, removing the RPZ obstruction. Jacobs
also developed the detailed civil design documents to include the demolition of the existing
shade hangars and associated paved hangar pads and taxiway connectors in PSO #34.
SCOPE OF WORK
Jacobs will provide Phase 1 Construction Support Services for the New Storage Hangars
project at The Florida Keys Marathon International Airport (MTH).
DESCRIPTION OF WORK
The project was advertised for bid on 1/11/16 and bids were opened on 3/2/16. Due to funding
limitations it was necessary to phase the execution of the project. Phase 1 will include all of the
site work and the procurement and delivery of the metal building and will take approximately
120 calendar days to complete. Phase 2 will include the erection and build out of the metal
building and Phase 3 the demolition of the existing shade hangars. Phases 2 & 3 will take
approximately 150 days and are contingent upon receiving additional funding.
The construction phase services will provide the following:
ARTICLE A — CONSTRUCTION PHASE SERVICES
Phase 1 Construction Support Services for the New Storage Hangars will be performed. The
specific tasks Jacobs will perform are:
Building
1. Prepare for and attend the Pre -Construction conference.
2. Respond to all Requests for Information (RFI's)
3. Review all shop drawing submittals for conformance to the plans and specifications and
respond to the CONTRACTOR and Airport with detailed summary of the applicability and
acceptability of the shop drawing.

4. Attend site visits and project construction meetings. Observe work in progress and provide
reports to Owner. Assume eight (8) site visits (Bi-Monthly) throughout the course of the
construction of Phase 1.
5. Attend weekly progress meeting, via conference call, when not in attendance at Bi-monthly
site visits. Assume eight (8) conference calls for progress meetings throughout the course
of the construction of Phase 1.
6. Discipline specific site visits; Structural.
7. Prepare and negotiate change orders, as necessary.
Civil
1. Prepare for and attend the Pre -Construction conference.
2. Issue the Notice to Proceed to the Contractor.
3. Respond to all Requests for Information (RFI's)
4. Review all shop drawing submittals for conformance to the plans and specifications and
respond to the CONTRACTOR and Airport with detailed summary of the applicability and
acceptability of the shop drawing.
5. Attend periodic site visits (up to four (4). Observe work in progress and provide reports to
Owner.
6. Attend weekly progress meetings via conference call. Assume sixteen (16) conference calls
for progress meetings throughout the course of the construction of Phase 1.
7. Prepare written or verbal directives to the Contractor
8. Coordination with QA Testing
9. Prepare field sketches
10. Review and approve contractor pay request
11. Prepare and negotiate change orders
12. Review QA Results
Services for Article A will be provided under a lump sum basis.
QUALIFICATIONS AND ASSUMPTIONS
For the purpose of providing a stipulated fee amount, this proposal has been based on the
following qualifications and assumptions:

■ Construction phase services are based on an anticipated duration of 120 calendar days
of construction in the field, Phase 1 only. In the event the construction duration exceeds
120 calendar days, Jacobs reserves the right to revise our proposal.
■ Jacobs Project Management Company will invoice on a percent complete, including
expenses, to compensate the level of effort required for each assignment.
■ Requests for Information (RFI's): It is assumed that not more than 35 RFI's will be
asked and responded to.
■ Construction support services are based upon the following: 13 shop drawings / product
submittals. Our assumption is 80% of submittals will be approved on the first review and
20% will require re -submittal for a total of 16 submittal reviews.
■ Full time or part time Resident Project Representative (RPR) is not included.
■ Phase 2 & 3 Construction Phase Services are not included.

TIERRA SOUTH FLORIDA (TSF), Inc. is pleased to submit the proposal for providing
construction material testing services for the above referenced project in Monroe County, Florida.
Our proposal is based on information provided to this office. We understand that the project in
general consists of new hangars consisting of pre-engineered metal buildings, new taxiway,
drainage, and site improvements. The following scope of services will be required at this project:
• Provide nuclear density tests on structural fill, backfill, subgrade and base materials, along
with LBR tests, gradation, Proctor tests, Atterberg Limits;
• Provide senior engineering technician for concrete and grout (Auger -cast -piles) sampling;
• Provide engineering technician for soils and concrete cylinder pick-up;
• Provide senior engineering technician to assist with asphalt plant inspections during
production (asphalt plant in Miami);
• Provide senior engineering technician to observe auger -cast -pile installation;
• Provide floor flatness testing;
• Provide Senior Engineer for report review, coordination, sign and seal reports.
TSF will provide experienced technical personnel to perform the necessary testing and inspection
services in accordance with the project specifications. We propose to accomplish the scope of our
services on a unit rate basis in accordance with the attached rate schedule. TSF's fees will be
determined by the actual amount of technical time expended for this project and the amount of
laboratory testing performed. We understand most services will be provided during day time.

AWS and FAA standards. Our laboratory maintains CMEC and FDOT certification and is staffed

We proposed to accomplish the above services on a rate schedule basis using the attached rates
with a not to exceed amount of $48,130.
