ANNUAL BIOMETRIC SCREENINGS: Four hours of annual leave to Monroe County BOCC employees and participating Constitutional Officer (Property Appraiser, Court Administration, and Land Authority) employees who participate in the annual biometric screenings performed at the health fair or visit their personal physician and complete the on-line Personal Health Assessment. Participation has been as follows since 2012. 2. Out of the approximate possible 500 BOCC employee responses, we received 113 (20%) individual responses to the survey. A summary of the results are as follows: # of responses Comment 18 I do not plan on participating in the Wellness Program 95 I plan on participating in the Wellness Program 4 I don't want anyone to know my personal health information 3 $25 per month is not enough money to incentivize my participation 1 I don't want to give up smoking 5 I don't want to spend time doing the preventative screenings 23 Would like paid gym memberships or free exercise programs at county buildings (such as Zumba, Yoga, etc.) A copy of the full survey results is attached (Attachment 1). A Frequently Asked Question (FAQ) document was developed and emailed to employees to address the various concerns and questions from the survey results (Attachment 2). Based on the survey results, we are now proposing a 3rd component of the Wellness Program for FY17/18 as follows: 3. GYM MEMBERSHIP REIMBURSEMENT PILOT PROGRAM: $25/month ($300/year) for up to the first 500 employees who sign up for a gym and exercise for a full year. There are a possible 1236 participants. Funding for this program will come from the County Wellness Fund, which currently has approximately $202,000. If all 500 employees participate the reimbursement costs would be $150,000. We are also investigating funding onsite Zumba, yoga, etc. at County buildings. Currently, there are some programs provided for employees for $5/employee/class. A more detailed description of the FY17/18 proposed programs follows: 1. ANNUAL BIOMETRIC SCREENINGS: BOCC and participating Constitutional Officer (Property Appraiser, Court Administration, and Land Authority employees (those covered and not covered by the County Health Insurance) who: A. Attend the annual Health Fair and participate in biometric, blood pressure, cholesterol, blood sugar level and body mass index screenings OR B. Visit their personal physician and obtain biometric, blood pressure, cholesterol, blood sugar level and body mass index screenings (on or between the dates of 11/1/16 to 10/31/17) and complete the on-line personal health assessment by 2/1/18. Will receive 4 hours of personal time. 2. ANNUAL WELLNESS EXAM AND OBTAIN THE REQUIRED AGE APPROPRIATE OR DOCTOR RECOMMENDED SCREENINGS: BOCC and Constitutional Officer employees who: A. Are covered under the health insurance who complete the Assessment Acknowledgement Form, which includes the requirement to visit their doctor between the dates of 11/1/16 to 10/31/17 for an annual wellness exam; and B. Obtain the required screenings based on age and doctor determination of need; and C. Attest to being a non -tobacco user Will obtain a $25 per month discount ($300 per year) on their health insurance premium to begin on January 1, 2018 until December 31, 2018. 3. GYM MEMBERSHIP REIMBURSEMENT PILOT PROGRAM: Beginning January 1, 2017, up to 500 BOCC and Constitutional Officer employees who: A. Obtain a 1-year gym membership contract and obtain preapproval from the Benefits office to sign up for membership; and B. Bring back the signed executed copy of the contract to the Benefits Office within 5 calendar days to confirm membership; and C. Utilize the gym, track their utilization, and attest to usage; and D. Submit receipts demonstrating 12 months of payments to the gym Will receive $300 annual reimbursement *Currently the Florida Blue 365 program has two participating gyms who offer a discounted membership fee of $25 per month (BodyZone Fitness and Forever Fit Studio — both located in Key West). We are working towards adding participating gyms in Marathon and Key Largo. It would be very helpful to be able I turn in required paperworkto our benefits office when we have ompleted whatever exam we have had done instead of havingto hold onto completed paperwork until everything is done. For instance, I had part of my required exams/tests done in October 201` and had to retain the paperwork for these exams until May 2016, when I completed the rest of the required exams/testsH had to wait until May to complete blood tests as I had done so in May 2014). So, had the original, signed paperwork from my doctors office for months before I was able to turn it all in, during which time it could easily have been misplaced. Section 17 of Laws 1984, c. 84-266, provides: "Notwithstanding any previous rejection or cancellation of participation in any group insurance plan or self- insurance plan provided under s. 110.123 or s. 112.0801, Florida Statutes, participation in such insurance plan shall be offered to all former employees who retired prior to the effective date of this act [July 1, 1984], as well as those employees who retire on or after such date, and the eligible dependents of such employees and former employees. Accordingly, all employers who provide insurance coverage under s. 110.123 or s. 112.0801, Florida Statutes, shall, nqt later than January 1, 1985, notify all previously retired former personnel or their eligible dependents in writing of their eligibility to participate in the same group insurance plan or self-insurance plan, as provided in s. 110.123 or s. 112.0801.
