At the July 20, 2016 Board of County Commissioner's meeting the Board granted approval and
authorized execution of Items:
C 1 Approval of Amendment 8 to the contract with Metric Engineering, Inc for Engineering Design
and Permitting Services for the US 1 Bayside Shared Use Path/Pedestrian Bridge Over Marvin Adams
Waterway Project to extend the expiration date of the contract to March 31, 2017 at no additional cost.
C2 Approval of Change Order 2 to the construction contract with Douglas N. Higgins, Inc for the
Lake Surprise Estates (Key Largo) Roadway and Drainage Improvements Project to incorporate sea
level rise adaptation and to adjust some quantities to actual vs "as bid" amounts for an additional cost of
$397,468.43.
C3 Approval to award bid and approve a contract with H&R Paving, Inc. For Construction of the
Sexton Cove Estates (Key Largo) Roadway and Drainage Improvements project in the amount of
$1,105,342.35.
C8 Approval of Amendment 1 to the contract with Metric Engineering Inc for Engineering Design
and Permitting Services for the Pigeon Key Ramp Restoration Project to transfer funds from the
Foundation Investigation task to the Geotechnical task for a lead survey, with no change to the original
total contract amount.
C 14 Approval of Amendment 1 to Agreement with T. Y. Lin International For Engineering Design
and Permitting Services for the Stock Island I Roadway and Drainage Improvement Project for
additional design services for the sea level rise adaptation initiative at an additional cost of $34,752.00.

THIS AMENDMENT 1 to the CONTRACT is entered into this 20th day of July, 2016 between Monroe County,
whose address is 1100 Simonton Street, Room 2-216 Key West, Florida 33040, hereafter the "COUNTY" and Metric
Engineering Inc., a Corporation of the State of Florida, whose address is 13940 S.W. 136"' Street, Suite 200, Miami, FL
33186, hereafter referred to as "CONSULTANT".
WHEREAS, on the 15th day of April, 2015 the parties executed a contract authorizing the Consultant to perform
Professional Services for the Design and Permitting Services for the Pigeon Key Ramp Restoration Project for a sum not
to exceed $205,959.71 (including $19,512.00 construction phase services); and
WHEREAS, a lead survey (as described in Attachment A) is required for the project which was not included in the
original CONSULTANT Geotechnical task, however there are funds remaining in the Foundation Investigation task and
therefore funds will be transferred from the Foundation Investigation task to the Geotechnical task; and
NOW THEREFORE, in consideration of the mutual promises contained herein, the COUNTY and CONSULTANT
agree to amend Paragraph 7.1.1 of the contract as follows:
Foundation Investigation task reduced $2,300.00 from $48,706.09 to 8 405.00.
Geotechnical Task for Lead Survey increased $2,300.00 from $19,695.00 to $21.105.00.
Total Contract Amount remains $205,959.71.
FORCE and EFFECT. In all other respects the Contract approved at the April 15, 2015 BOCC meeting remains in full
force and effect.

Dear Mr. Ruggiano:
Professional Service Industries, Inc. (PSI) is pleased to present this proposal to perform a
lead -based paint inspection of the Pigeon Key Ramp located in Pigeon Key, Florida.
Background
PSI understands the survey will be performed on the bridge structure that is scheduled
to be renovated. The bridge is approximately 262 linear feet in length.
Scope of Work
The purpose of the lead -based paint survey is to assist the facility owner and contractor
performing the renovation in complying with requirements of OSHA 29 CFR 1926.62 —
Lead in Construction, Resource Conservation and Recovery Act (RCRA), Toxic
Substances Control Act (TSCA), and the Universal Waste Rule (UWR) governing the
disposal of hazardous waste.
PSI will collect paint chip samples to assess the potential presence of LBP hazards that may
exist on the affected bridge components. PSI will collect one paint chip sample from each of the
different bridge components observed. All samples will be analyzed for total lead using Flame
Atomic Absorption Method. (SW 846 3050B*/7000B).
Detailed records will be kept describing each sampling location selected, the condition of the
substrate and paint for each sample, and anything unusual about the testing conditions.
Final Report
PSI will provide two copies of a final report for the bridge within fifteen working days from
the time of the survey, provided no delays such as access to the structure are incurred.
The letter report will summarize field observations and laboratory analytical results.
Project Schedule & Cost Estimates
It is proposed that the fee for the performance of the outlined services be determined on
a lump sum basis for the lead -based paint survey, including the collection and analysis of
up to 10 paint chip samples.
