This Amended Task Order is entered into on the effective date noted above pursuant to the
"Master Agreement for Professional Services" between Monroe County, Florida ("Client") and
JACOBS PROJECT MANAGEMENT CO. ("Consultant"), dated November 16, 2011
("Agreement") as ammended by Amendment #1, dated October 17, 2012 ("Addendum"). The
Agreement is incorporated herein and forms an integral part of this Task Order.
Services Authorized: Adds MTH New Storage Hangars - Phase 2 & 3 Construction
Support Services in the amount of $117,300.00 increasing the total fixed price for this
project to $252,550.00
Client authorizes Consultant to perform the Services described in Exhibit A attached hereto and
incorporated herein, which Exhibit A is marked with the above noted Task Order No. and
consists of 3 page(s).

New Storage Hangars — Construction Administration Phases 2 & 3
EXISTING CONDITIONS
The eleven shade hangars on the northeast side of RW 25 along Taxiway E have exceeded
their useful life span and are also within the Runway Protection Zone (RPZ) of Runway 25.
These shade hangars will be demolished after the construction of the eleven new hangars on
the southeast side of RW 25 along Taxiway A, therefore, removing the RPZ obstruction. Jacobs
also developed the detailed civil design documents to include the demolition of the existing
shade hangars.
SCOPE OF WORK
Jacobs will provide Phase 2 & 3 Construction Administration Services for the New Storage
Hangars project at The Florida Keys Marathon International Airport (MTH).
DESCRIPTION OF WORK
The project was advertised for bid on 1/11/16 and bids were opened on 3/2/16. Due to funding
limitations it was necessary to phase the execution of the project. Phase 1, which is currently
under construction, includes all of the site work and the procurement and delivery of the metal
building and will take approximately 120 calendar days to complete. Jacobs Construction
Administration services, for only Phase 1, were awarded under a previous PSO. Phase 2 will
include the erection and build out of the metal building and Phase 3 the demolition of the
existing shade hangars. Phases 2 & 3 combined will take approximately 150 days.
The construction phase services for Phase 2 & 3 will provide the following:
ARTICLE A — CONSTRUCTION PHASE SERVICES
Phase 2 & 3 Construction Administration for the New Storage Hangars will be performed. The
specific tasks Jacobs will perform are:
Building
1. Respond to all Requests for Information (RFI's)
2. Review all shop drawing submittals for conformance to the plans and specifications and
respond to the CONTRACTOR and Airport with detailed summary of the applicability and
acceptability of the shop drawing.

3. Attend site visits and project construction meetings. Observe work in progress and provide
reports to Owner. Assume ten (10) site visits (Bi-Monthly) throughout the course of the
construction of Phase 2 & 3.
4. Attend weekly progress meeting, via conference call, when not in attendance at Bi-monthly
site visits. Assume ten (20) conference calls for progress meetings throughout the course of
the construction of Phase 2 & 3.
5. Discipline specific site visits; Mechanical, Electrical and Plumbing.
6. Prepare and negotiate change orders, as necessary.
7. Prepare and submit Record Drawings.
8. Attend Final Inspection and prepare Punchlist.
9. Prepare and submit closeout and warranty package.
Civil
1. Respond to all Requests for Information (RFI's)
2. Review all shop drawing submittals for conformance to the plans and specifications and
respond to the CONTRACTOR and Airport with detailed summary of the applicability and
acceptability of the shop drawing.
3. Attend periodic site visits up to four (4). Observe work in progress and provide reports to
Owner.
4. Attend weekly progress meetings via conference call. Assume twenty (20) conference calls
for progress meetings throughout the course of the construction of Phase 2 & 3.
5. Prepare written or verbal directives to the Contractor
6. Prepare field sketches
7. Review and approve contractor pay request
8. Prepare and negotiate change orders
9. Prepare and submit Record Drawings.
10. Attend Final Inspection and prepare Punchlist.
11. Prepare and submit closeout and warranty package.
12. Prepare Engineer of Record certifications to permitting agencies.
Services for Article A will be provided under a lump sum basis.

QUALIFICATIONS AND ASSUMPTIONS
For the purpose of providing a stipulated fee amount, this proposal has been based on the
following qualifications and assumptions:
■ Construction phase services are based on an anticipated duration of 150 calendar days
of construction in the field, Phases 2 & 3. In the event the construction duration exceeds
150 calendar days, Jacobs reserves the right to revise our proposal.
■ Jacobs will invoice on a percent complete, including expenses, to compensate the level
of effort required for each assignment.
■ Requests for Information (RFI's): It is assumed that not more than 35 RFI's will be
asked and responded to.
• Construction support services are based upon the following: 13 shop drawings / product
submittals. Our assumption is 80% of submittals will be approved on the first review and
20% will require re -submittal for a total of 16 submittal reviews.
■ Full time or part time Resident Project Representative (RPR) is not included.
