At the October 21, 2015 Board of County Commissioner's meeting the Board granted approval and
authorized execution of Item Q5 Amendment No. 5 to the contract with AMEC Foster Wheeler
Environment and Infrastructure, Inc. for the Engineering Design, Permitting, Field Assessments, Project
Management and Engineering Support Services for the Canal Demonstration Projects to extend the
scope of services at Canal #290 in the Avenues in Big Pine Key for additional Construction
Administration and Construction Engineering Inspection Services.

AMENDMENT NO. 5
TO THE CONTRACT
FOR
ENGINEERING, DESIGN AND PERMITTING SERVICES
FOR THE
CANAL DEMONSTRATION PROJECTS
THIS AMENDMENT NO. 5, dated October 21, 2015, is entered into between the County and
the CONSULTANT, to the Contract For Engineering, Design And Permitting Services, dated the
21' day of May, 2014, as amended September 17, 2014 (Al), February 18, 2015 (A2), May 20,
2015 (A3), and July 15, 2015 (A4) by and between Monroe County Board of County
Commissioners, "COUNTY," and AMEC Foster Wheeler Environment & Infrastructure, Inc.,
"AMEC ".
WITNESSETH:
WHEREAS, much engineering and administrative coordination work has been performed under
the Contract for design and construction oversight of the canal demonstrations projects; and
WHEREAS, the organic removal project construction contractor JND Thomas has requested
additional time under their construction contract to allow for the removal of muck within a five
foot buffer zone of each canal side, which was at the request of the homeowners, and would also
provide a more effective water quality improvement; and
WHEREAS the schedule for construction, which includes Saturday work, has been extended
accordingly; and
WHEREAS, additional funds are required to fund the additional CEI onsite inspections by
AMEC for the additional time needed; and
WHEREAS, an extension of time is required to extend the AMEC contract from December 31,
2015 through February 29, 2016;
NOW, THEREFORE, in consideration of mutual promises, covenants and contracts stated
herein, and for other good and valuable consideration, the sufficiency of which is hereby
acknowledged, COUNTY and AMEC agree as follows:
1. AMEC will provide the scope of services as specified in Exhibit "A -5" attached and made a
part of this AMENDMENT No. 5.
2. This AMENDMENT No. 5 provides an additional amount not to exceed Fifty Nine Thousand
Two Hundred Twenty Two Dollars and Eighty Six Cents ($59,222.86) for additional CEI
construction support services of the organic removal project on canal #290 on Avenue J in Big
Pine Key. The overall CONTRACT value is increased from Seven Hundred Twenty Five
Thousand Two Hundred Twenty Three Dollars and Twenty Eight Cents ($725,223.28 to Seven
Hundred Eighty Four Thousand Four Hundred Forty Six Dollars and Fourteen Cents
($784,446.14.)

3. Travel expenses are updated to reflect the revisions to the Monroe County rates. The mileage
allowance rate of $0.53 per mile and the subsistence (meal) allowance rates for Breakfast of
$6.50, Lunch of $13.50 and Dinner of $27.00 are authorized.

AMEC FOSTER WHEELER'S SCOPE OF SERVICES
A. Construction Support Services And Engineering During Construction At Canal #290
Organic Removal Construction
1. Scope of Work:
The AMEC Foster Wheeler's scope of work associated with the original contract for Engineering,
Design and Permitting Services for Canal Demonstration Projects dated May 21, 2014 included
work at Canal #290 Big Pine Key the Avenues Organic Removal related to Construction Support
Services and Engineering during Construction which was identified as Task 7. The scope included
daily site inspections and construction administration for the organic removal project. The Organic
Removal project construction contractor JND Thomas has requested additional time under their
construction contract to allow for the removal of muck within a five foot buffer zone of each canal
side, which was at the request of the homeowners, and would also provide a more effective water
quality improvement. The contractor also requested to extend the work week to include Saturdays.
JND Thomas provided an updated schedule which extended their construction completion date to
February 5, 2016. Additional funds are required to fund the additional CEI onsite inspections by
AMEC for the additional time needed by the contractor to complete the project. An extension of
time is also required to extend the AMEC contract from December 31, 2015 through February 29,
2016;
a) Site Inspections: AMEC will oversight the contractor for up to 4 hours a day for Monday
through Saturday field construction activities through February 5, 2016 and prepare daily
work reports showing contractors activities, equipment, maintenance of traffic and
environmental compliance.
b) Construction Administration - AMEC will provide consultation and advice to County
regarding the Contractor's performance of the Contract. AMEC will liaise with the County,
homeowners, and other stakeholders as necessary to administer the construction contract
through the estimated Feb 5, 2016 project completion date.
2. Deliverables:
• Daily work reports
• Progress pay estimates with back up documentation
• Final Pay Estimate with back up documentation
• Final "As -Built Plans"
• Material certifications
• Environmental Inspection Reports
• Progress and Final Photographs (Electronic Files).
3. Budget: $ 59,222.86
A detailed cost spreadsheet is attached. Additional staff may be utilized as per the contract rate
schedule.
4. Estimated Completion Date: Total Project Completion date of February 5, 2016.

From time to time, Amec Foster Wheeler retains outside Professional and Technical labor on a
