AGENDA ITEM WORDING: Approval of a Professional Services Agreement between Monroe
County and Health Designs, Inc. for Health Designs to provide a Wellness Champion that will travel
to all County office locations and promote wellness and ways to utilize health and prescription
insurance benefits in a more prudent way to save money for the insurance user, as well as the
County.
ITEM BACKGROUND:
The County has been working toward providing increased education related to wellness, as well as
use of health and prescription benefits. County benefits staff are focused on operations of all county
benefits and are located in Key West.
To increase awareness and to help educate employees of the County, as well as the constitutional
officers, employees, we are requesting approval to engage the services of a consultant with expertise
in health benefits to serve as an "Onsite Wellness Champion ".
The concept is for the consultant to visit every County Office Building every quarter, when the
employees are most likely to attend and receive training about the features of the Monroe County
Wellness Program and the health and prescription plan with a focus on how to save money. Exhibit
A attached to the contract shows the scheduled quarterly site visits beginning July 2017. Each visit
will include: 1) 30 Minute Wellness Topic Discussion; 2) Recap of the MCBOCC Passport to
Wellness Program Guidelines /Incentives /Countdown and 3) MCBOCC Health Plan Education Items
(items will be provided by Benefits Office).

Wellness participation in the Health Fairs has increased steadily over the past several years as
follows:
PREVIOUS RELEVANT BOCC ACTION: The MCBOCC approved a Wellness Program in
2016 creating the opportunity for employees to save $300 annually on their health insurance
premium starting January 1, 2017, by having the age appropriate preventive screenings and have
their physician complete the Employee Health Preventive Care — Assessment Acknowledgement
Form. The program was approved by the Board to continue into 2017. The engaging of Health
Designs to promote our Wellness Program along with educating of our employees on our health plan
will get our participants more involved in their health care program and hopefully make smarter
choices when utilizing their health benefits.

Professional Services Agreement by and between

WHEREAS, the parties desire to enter into an agreement for the services required
