Dear Mr. Willy:
The Health Council of South Florida Inc. (Council) is pleased to submit the
application of Debra Walker, Ph.D. to the Board of Directors in the category of
Consumer for a two year term. The seat became vacant due to the passing of
William Kwalick. The Council's Nominating Committee convened to review
applications in accordance with Council Bylaws and recommends the appointment
of Dr. Walker to the Council's Board of Directors. Dr. Walker's application form and
resume are attached.
The Council also requests the reappointment of Kerry Shelby, C,P.A. to a two
year term in the category of Purchaser. His current term expires on October 14,
2005. The Council's Nominating Committee reviewed Mr, Shelby's status and
determined that he is in good standing, Mr, Shelby is also a member of the
Board's Finance Committee. His application form and resume are enclosed,
If possible, we respectfully request that these applications be submitted to the
Commissioners for appointment at the next County Commission Meeting. Please
